PCT、IL-6和hs-CRP在新生儿感染性疾病中的诊断价值

摘　　要：目的探讨降钙素原(PCT)、白细胞介素6(IL-6)和超敏C反应蛋白(hs-CRP)在新生儿感染性疾病早期诊断中的应用价值。方法对正常对照组36例、感染性疾病新生儿童60例,采用化学发光法测定血清中PCT与IL-6,用免疫比浊法测定血清中hs-CRP水平。结果感染组急性期PCT、IL-6和hs-CRP 3项检测指标分别为(3.62±2.55)μg/L、(28.8±10.5)ng/L和(14.8±2.9)mg/L,恢复期3项检测指标分别为(0.038±0.01)μg/L、(4.9±2.1)ng/L和(3.9±2.03)mg/L;对照组3项检测指标分别为(0.036±0.008)μg/L、(4.63±1.98)ng/L和(3.8±1.9)mg/L。感染组急性期3项指标明显高于其恢复期和对照组(P<0.01)。三者相比,PCT和hs-CRP的特异性高于IL-6,IL-6的敏感性高于PCT和hs-CRP(P<0.01)。结论 PCT、IL-6和hs-CRP动态检测对新生儿感染性疾病早期诊断及疗效判定具有一定价值。[Objective]To explore the value of procalcitonin（PCT）,interleukin-6（IL-6） and high sensitivity c-reactive protein（hs-CRP） in early diagnosis of neonatal infectious diseases.[Methods]The control group included 26 healthy objects,and the infectious group included 60 cases of neonatal infectious diseases.The levels of serum PCT and IL-6 were detected by chemiluminescencem,and the level of serum hs-CRP was detected by immune turbidimetry.[Results]In the infectious group,the level of serum PCT,IL-6 and hs-CRP in acute stage was respectively（3.62±2.55） μg/L,（28.8±10.5）ng/L and（14.8±2.9） mg/L,and that in restoration stage was（0.038±0.01） μg/L,（4.9±2.1） ng/L and（3.9±2.03） mg/L respectively.The level of serum PCT,IL-6 and hs-CRP in the control group was respectively（0.036±0.008）μg/L,（4.63±1.98）ng/L and（3.8±1.9）mg/ L.The levels of serum PCT,IL-6 and hs-CRP in acute stage in the infectious group were significantly higher than those in restoratio stage and in the control group.The specific of PCT and hs-CRP was higher than that of IL-6,and the sensitivity of IL-6 was higher than that of PCT and hs-CRP（P0.01）.[Conclusion]The dynamic detection of PCT,IL-6 and hs-CRP has certain value in early diagnosis and curative effect assessment of neonatal infectious diseases.
