臭氧髓核溶解术治疗椎间盘源性腰痛临床效果评价

摘　　要：目的观察臭氧髓核溶解术与保守治疗2种模式治疗椎间盘源性腰痛的临床效果,探索最佳治疗方法。方法选取自2009年5月—2011年5月在秦皇岛市第三医院确诊为椎间盘源性腰痛的患者157例,随机分为观察组与对照组,观察组78人接受臭氧髓核溶解术,对照组79人接受保守治疗,2组患者治疗均获为期1 a的随访,采用改良MacNab疗效评价标准以及Oswestry功能障碍指数评价临床疗效。结果 1 a随访期内,根据MacNab腰腿痛评价标准,观察组患者在术后第1、2、3、6、9和12个月各时间点的有效率均明显高于对照组(87.2%vs 72.2%、89.7%vs 69.6%、91.0%vs 68.4%、89.7%vs 68.4%、88.5%vs 65.8%、85.9%vs 65.8%),差异均有统计学意义(P<0.05或P<0.01)。根据Oswestry功能障碍指数,观察组术后12个月评分明显低于对照组[(28.285±13.120)vs(43.542±12.952)],差异亦有统计学意义(P<0.01)。结论臭氧髓核溶解术治疗椎间盘源性下腰痛临床疗效显著,恢复良好。[Objective]To observe the clinical effect of ozone chemonucleolysis and conservative treatment for discogenic low back pain,explore the best treatment.[Methods]157 patients who were diagnosed with discogenic low back pain in Third Hospital of Qinhuangdao City from May 2009 to May 2011 were randomly divided into the observation group and the control group.The observation group（78 cases） was treated with ozone chemonucleolysis,the control group received conservative treatment,and two groups were given 1 year follow-up.The clinical efficacy was evaluated by MacNab Efficacy Evaluation Criteria and the Oswestry Disability Index.[Results]During the 1-year follow-up period,according to the MacNab Evaluation Criteria of low back pain,the effective rate of the observation group at 1st,2nd,3rd,6th,9th and 12th month after operation was higher than that of the control group（87.2% vs 72.2%,89.7% vs 69.6%,91.0% vs 68.4%,89.7% vs 68.4%,88.5% vs 65.8%,85.9% vs 65.8%） significantly（P0.0 or P0.01）.According to the Oswestry Disability Index,the score in 12 months after operation of the observation group（28.285±13.120） was lower than the that of the control group（43.542±12.952） significantly（P0.01）.[Conclusion]Ozone chemonucleolysis has obvious clinical efficacy for discogenic low back pain,and patients have a good recovery.
