摘  要:目的探讨含1%布替萘芬和0.05%莫米松的复方乳膏制剂的体外抗真菌谱与抗真菌活性,并与已经上市的抗真菌药糖皮质激素复方制剂曲安奈德益康唑乳膏、复方克霉唑乳膏及单纯制剂盐酸布替萘芬乳膏、莫米松乳膏做平行对照研究。方法选用临床分离致病真菌及马拉色菌标准株共13种24株,用琼脂扩散法作药敏试验。将各种培养基制成含2%琼脂的含菌平板,打孔后分别加入上述5种乳膏,培养7天后测定各含药孔周围的抑菌圈直径。结果布替萘芬莫米松乳膏对常见皮肤癣菌、申克孢子丝菌、裴氏着色真菌、马拉色菌等10种16株菌株形成抑菌圈,对各念珠菌酵母态和白念珠菌菌丝态无抑菌圈;对于申克孢子丝菌菌丝相的抑菌圈总体上大于曲安奈德益康唑乳膏和复方克霉唑乳膏;对常见皮肤癣菌的的抑菌圈总体上等于曲安奈德益康唑乳膏,大于复方克霉唑乳膏;对于裴氏着色真菌和申克孢子丝菌酵母相,小于曲安奈德益康唑乳膏,大于复方克霉唑乳膏;对于马拉色菌,小于曲安奈德益康唑乳膏与复方克霉唑乳膏,其差异均有统计学意义(P<0.05)。结论布替萘芬莫米松乳膏与盐酸布替萘芬乳膏抗菌谱一致,对于申克孢子丝菌菌丝相、皮肤癣菌的抑菌作用优于、等同于曲安奈德益康唑乳膏和复方克霉唑乳膏,对于裴氏着色真菌和申克孢子丝菌酵母相,优于复方克霉唑乳膏,可作为急性浅部真菌感染和继发真菌感染的皮炎湿疹类疾病的治疗选择。Objective To compare the antifungal spectrum and activity of butenafine and mometasone (BM) cream in vitro with butenafine hydrochloride (BH), triamcinolone acetonide and econazole nitrate (TAEN) , betamethasone dipropionate clotrimazole and gentamycin sulfate (BDCG) and mometasone furoate creams against isolates of the common dermatophytes, Sporothrix schenckii (S. schenckii) , Candida spp. , Malassezia spp. and Fonsecaea pedrosoi (F. pedrosoi ) using a modified agar diffusion assay suitable for clinical use. Methods Twenty-one pathogenic fungi from ten species isolated from patients and three species of Malassezia standard strains were used in the study. A modified agar diffusion assay was performed using the disk diffusion susceptibility method. Growth sta- tus of the isolates and the diameters of the inhibition zones around drug-containing wells were recorded using photography following a sev- en-day incubation period. Results The BM cream inhibition zones were observed against sixteen test isolates of the common dermato- phytes, S. schenckii, Malassezia spp. and F. pedrosoi, while no inhibition zones were around the BM cream in isolates of Candida spp. The BM cream inhibition zones against S. schenckii, dermatophytes and F. pedrosoi were significantly larger than (P〈O. 05) or equal to (P〉0.05) those of TAEN, BDCG creams. Conclusion The in vitro spectrum of the BM cream was consistent with that of the BH cream. Compared with the other two marked compound creams with antifungal drug and glucocorticoid (TAEN and BDCG creams) , the BM cream displayed stronger or equal i'n vitro antifungal activity against clinical isolates of S. schenckii, dermatophytes and F. pedrosoi .
