摘  要:目的:探讨CD40分子与颈动脉粥样硬化斑块的关系以及对预后的影响。方法:160例颈动脉粥样硬化患者均行CD40分子检测和颈动脉超声检查,按照颈动脉超声检查的结果分为:软斑组(88例)和硬斑组(72例),统计分析两组颈动脉粥样硬化斑块与CD40分子的表达水平关系,随访观察两组3年内脑梗死的发生率。结果:与硬斑组比较,软斑组CD40分子水平明显升高[(2.88±1.34)g/L比(4.02±1.17)g/L,P<0.05],3年内脑梗死发生率亦明显升高(1.39%比9.09%,P<0.05)。结论:CD40是炎症反应中的重要信号分子,其表达的高低可用来评估颈动脉粥样硬化斑块的风险,指导早期脑梗死的预防治疗。Objective: To investigate relationship between CD40 molecule and carotid atherosclerotic plaque and its influence on prognosis. Methods: A total of 160 cases with carotid atherosclerosis underwent CD40 molecule detection and carotid ultrasound measurement. According to results of carotid ultrasound measurement, patients were divided into soft plaque group (n = 88) and hard plaque group (n = 72). Expression levels of CD40 molecule of two groups were compared and they were followed up for incidence rate of cerebral infarction within three years. Results: Compared with hard plaque group, there were significant increase in level of CD40 molecule [ (2.88± 1.34) g/L vs. (4. 02 ± 1.17) g/L, P〈0.05] and incidence rate of cerebral infarction within three years (1.39% vs. 9. 09%, P〈0.05) in soft plaque group. Conclusion: CD40 is an important signal molecule in inflammatory response, and its expression level can be used to assess risk of carotid atherosclerotic plaque and guide prevention and treatment of early cerebral infarction.
