胃癌前病变中医证型与甲皱微循环的相关性研究

摘　　要：[目的]研究胃癌前病变患者的中医证型与甲皱微循环改变的相关性。[方法]治疗组65例患者,以及健康对照组62例,采用XW880型微循环检测仪,选取左手无名指甲襞边缘涂擦香柏油后,进行甲襞微循环检查,并按田牛提出的甲襞微循环加权积分要求对管袢形态、血液流态、袢周状态及总积分进行评价。[结果]胃癌前病变患者在甲襞微循环管袢形态、管袢流态、管袢状态方面存在不同程度异常,两组经统计学分析,P<0.01;且以肝胃不和、脾胃湿热、胃络瘀血的总积分值为高,与其他组比较,P<0.05。[结论]血液高凝状态是影响胃黏膜细胞萎缩变性的主要因素,肝失疏泄,气机不畅,脾胃湿热内蕴对胃黏膜微循环的影响较大,也可能具有更大的癌变趋势,临床治疗中要注意疏肝解郁、清利湿热药物的合理选用,可提高临床疗效。[Objective] To observe the correlation of precancerous lesions TCM syndrome and nail fold microcirculation changes.[Method] 65 patients and the healthy control group 62 cases,the XW880 type microcirculation detector,select the left hand to nameless nail fold edge rubbed cedar oil,the nailfold microcirculation check press Tian Niu nailfold microcirculation the cycle weighted integral requires to evaluate pipe loop morphology,blood flow pattern,state loops weeks and the total score.[Results] Precancerous lesions in the the nailfold microcirculation tube loop morphology,pipe loop flow pattern,pipe loops there are varying degrees of abnormal state,the two groups by statistical analysis,P0.01;and disharmony of liver and,spleen,the stomach meridian bruises total integral value is high compared with other groups,P0.05.[Conclusion] Hypercoagulability main factors affecting gastric mucosal cells atrophy degeneration,liver failure catharsis,gas-poor,stomach damp gastric microcirculation,may also have a greater cancer trends pay attention to clinical liver qi stagnation,Dampness and Heat reasonable choice of drugs to improve clinical outcomes.
