摘 要:目的探讨能谱CT单能量成像对改善强直性脊柱炎髋关节病变图像质量的作用。方法对71例临床证实的强直性脊柱炎髋关节病变患者采用能谱CT成像模式扫描,获得常规混合能量图像和110keV单能量图像,对两种图像分别进行图像质量评分,记录可观察到的髋臼囊变数量和测量信噪比(CNR)。结果 110keV单能量图像的图像质量评分、髋臼囊变数量和信噪比均高于混合能量图像[分别为(4.48±1.54)和(3.74±1.47),t=10.32;(2.15±1.22)和(1.76±0.65),t=3.69;(5.56±3.72)和(3.88±2.71),t=9.24],各项指标比较,差异均有统计学意义(P<0.01)。结论能谱CT110keV单能量图像能够明显改善髋关节图像质量,有利于提高对强直性脊柱炎髋关节病变的各种征象的观察,尤其是可以提高髋臼囊变的检出率。Objective To explore the role of monochromatic spectral CT imaging in improving the image quality of lesion in ankylosing spondylitis. Methods 71 patients who've been clinically diagnosed with lesion in ankylosing spondylitis were scanned by spectral CT imaging to obtain conventional polychromatic images and 110keV monochromatic images which were respectively graded in terms of quality.Meanwhile,observable cystic changes in the acetabulum and signal to noise ratio(SNR) were recorded. Results The quality score,cystic changes in acetabulum and SNR of 110keV monochromatic images were higher than those of polychromatic images,the value of which were respectively shown as follows:(4.48±1.54),(3.74±1.47),t=10.32;(2.15±1.22),(1.76±0.65),t=3.69;(5.56±3.72),(3.88±2.71),t=9.24.After comparing all indicators,it was found that all differences were of statistic significances(P0.01). Conclusion 110keV monochromatic CT images can obviously improve the imaging quality of hip joint and they are favorable for observing the symptoms of the lesion in ankylosing spondylitis,particularly improving the detection rate of cystic changes in acetabulum.
