耐多药肺结核患者T淋巴细胞亚群测定的临床意义

摘　　要：目的探讨肺结核-耐多药(MDR-TB)患者外周血中活化T细胞的水平,对治疗的意义。方法对2006年11月—2011年11月的MDR-TB患者72例,初治菌阳性患者102例临床资料进行回顾性分析。结果在治疗前A组患者的外周血中,其CD3+值与CD4+值,以及其CD4+/CD8+比值,同B组比较,差异有统计学意义(P<0.05),但两组CD8+比较,无统计学差异(P>0.05);A组予以2个月的抗结核强化治疗后,有66例患者病情好转,并且症状显著改善,有56例患者痰菌阴转,并且其中50例患者复查后显示,其CD3+以及CD4+/CD8+比值同治疗前比较,上升明显(P<0.01),CD4+/CD8+治疗前后比较,无统计学差异(P>0.05),其余6例患者出现病灶增多,并且CD3+、CD4+、CD8+值,包括CD4+/CD8+比值,均较前降低。结论掌握MDR-TB患者外周血中其活化T细胞的水平,有助于依据患者具体情况实施个性化的治疗方案,具有非常重要的临床意义。Objective To investigate the level of activated T cells in the peripheral blood from patients with multidrug-resistant tuberculosis（MDR-TB） and its significance in treatment. Methods The data of 72 MDR-TB patients since November 2006 to November 2011 and 102 cases of governance smear positive patients were retrospectively analyzed. Results In peripheral blood from patients of group A before the treatment,the values of CD3＋ and CD4＋ and the ratio of CD4＋/CD8＋ were compared with those of group B,the difference was significant（P0.05）.But the difference between CD8＋ of two groups showed no statistic difference（P0.05）.Group A received 2 months of anti-TB intensive treatment,66 patients＇ condition improved and symptoms were improved significantly.56 patients showed sputum negative conversion,of which 50 patients＇ CD3＋ and the ratio of CD4＋/CD8＋ increased compared with pre-treatment（P0.01）.No statistic difference in pre-treatment and post-treatment CD4＋/CD8＋（P0.05）.Lesions increased in the remaining 6 patients,the values of CD3＋,CD4＋ and CD8＋ including the ratio of CD4＋/CD8＋ reduced. Conclusion Controlling of the level of activated T cells in the peripheral blood of patients with MDR-TB is helpful to making personalized treatment programs and has a very important clinical significance.
