常规体外受精失败后行补救性卵胞浆内单精子注射的临床价值探讨

摘　　要：目的：探讨对常规体外受精（IVF）周期受精完全失败患者，于授精后18—20h行补救卵胞浆内单精子注射（LATE—ICSI）的临床价值。方法：回顾分析2008年3月至2010年7月于佛山市妇幼保健院生殖中心行常规IVF后受精完全失败并进行LATE—ICSI的26个周期。分析项目包括受精率、卵裂率、可利用胚胎率、妊娠率以厦妊娠结局。结果：在26例常规IVF受精完全失败周期中，共荻卵256个，行补救ICSI的MII卵221个，受精率73．43％，卵裂率95．74％，可利用胚胎率83．51％，26例LATE--ICSI新鲜胚胎移植中有7例获得临床妊娠，临床妊娠率为26．92％，其中流产3例，流产率为42．9％，成功分娩3倒，共分娩健康新生儿4名。结论：对常规IVF受精完全失败周期，于受精后18—20h行补救ICSI，具有一定的临床应用价值。Objective： To assess the clinical value of late intracytoplasmic sperm injection （LATE--ICSI） in 18 to 20 hours after total conventional in--vitro fertilization（IVF） failure. Methods：Do research on base of 26 cases of LATE--ICSI after total conventional IVF failure , which happened during March 2008 to July 2010 in Foshan maternal and child care hospital. The analysis of the project including the rate of fertilization,merogenesis, available embryo,pregnancy. Result： In these 26 cases, 256 eggs are got and 221 MII eggs are applied to LATE-ICSI. The Statistical data as fol- lows： 73.43% for fertilization rate, 95.74% for merogenesis rate, 83. 51% for available embryo rate. In these 26 cases of embryo transfer after LATE--ICSI , 7 cases are clinical pregnancy, which means the clinical pregnancy rate is 26.92%. In these 7 cases, abortion for 3, which means the abortion rate was 42.9 %. And 3 case are complete success and get 4 healthy babies. Conclusion：It＇s valuable to apply late intracytoplasmic sperm injection （LATE--ICSI） in 18 to 20 hours after total conventional in--vitro fertilization（IVF） failure.
