老年慢性呼吸衰竭患者血小板活化因子及血小板功能改变

摘　　要：目的　探讨老年慢性呼吸衰竭患者血小板活化因子 (PAF)及血小板粘附、聚集、释放功能的改变及与老年慢性肺心病和呼吸衰竭发展的关系。　方法　生物定量法观察老年慢性呼吸衰竭组患者 (5 5例 )、老年健康人组 (40例 )及非老年健康人组 (35例 )血小板激活因子 (PAF)的改变 ,放免法观察血小板膜 GMP140、血小板释放功能、血小板粘附功能 (PADT)和血小板聚集功能 (PAg T)的变化。　结果　老年慢性呼吸衰竭组 PAF为 (171± 5 1) nm ol/ L 高于老年健康人的 (132± 40 ) nmol/ L(P<0 .0 1) ,老年健康人组也显著高于非老年健康人组 (P<0 .0 5 )。老年慢性呼吸衰竭患者 PADT、PAg T明显高于老年健康人组 (P<0 .0 1)及非老年健康人组 (P<0 .0 1)。老年慢性呼吸衰竭患者血小板膜 GMP140为 (4813± 180 8)分子数 /血小板 ,显著高于老年健康人组 (P<0 .0 1)及非老年健康人组 (P<0 .0 1)。　结论　慢性呼吸衰竭患者中 PAF产生和释放增加促使血小板功能亢进 ,进一步引起微血栓形成及肺循环障碍 ,促进老年慢性肺心病及呼吸衰竭的发展。Objective Platelet activating factor (PAF) plays a key role in platelet activation.During the process of aging there will be dysfunction of endothelium and thus inducing abnormal function of platelet and thrombosis. Our purpose is to investigate the change of PAF in chronic respiratory failure and its effect on platelet function. Methods PAF in plasma was detected and the function of platelet was determined by platelet adhesion test (PADT),platelet aggregation test (PAgT) and platelet grarule membrane (GM140) measurement in 55 elderly patients with chronic respiratory failure, 40 cases of healthy elderly subjects and 35 cases of healthy non elderly subjects. Results PAF level in the elderly patients with chronic respiratory failure was significantly higher than that in healthy elderly subjects ( P <0 05) and healthy non elderly subjects( P <0 01). PADT and PAgT results of the elderly patients with chronic respiratory failure were also higher than those in healthy elderly subjects ( P <0 01) and healthy non elderly subjects ( P <0 01). The GMP140 content on platelet membrane in the elderly patients with chronic respiratory failure was statistically higher than that in healthy elderly subjects ( P <0 01) and healthy non elderly subjects ( P <0 01). Conclusions The increase of PAF in elderly patients with chronic respiratory failure has a potent effect on the function of platelet and results in thrombosis and dysfunction of pulmonary circulation.
