地佐辛复合左旋布比卡因应用于臂丛神经阻滞的临床研究

摘　　要：目的观察地佐辛复合左旋布比卡因用于臂丛神经阻滞对麻醉和镇痛效果的影响。方法将40例需臂丛神经阻滞行上肢手术的患者随机均分成A、B组(每组20例),A组患者使用的是局麻药为0.5%的左旋布比卡因20 ml,A组不加地佐辛,B组患者使用的是局麻药为0.5%的左旋布比卡因20 ml中加入地佐辛0.05 mg/kg。然后分别记录两组的麻醉起效时间、痛觉消失时间、镇痛持续时间,同时采用VAS评分评价各组镇痛效果。不良反应:观察两组患者术后36 h的头痛、头晕、恶心、呕吐、皮肤瘙痒、呼吸抑制、尿潴留等不良反应发生情况。结果 A组镇痛持续时间明显短于B组(P<0.05),B组麻醉后3,6,9 h静息VAS评分显著低于A组(P<0.05)。A组术后有10例需肌注杜冷丁,而B组无一例。所有患者术中、术后均未见恶心、呕吐、皮肤瘙痒、呼吸抑制、尿潴留等并发症。结论地佐辛复合左旋布比卡因应用于臂丛神经阻滞能够延长麻醉镇痛持续时间,不良反应少。Objective To observe the effect of levobupivacaine hydrochloride together with dezocine on brachial block. Meth- ods Forty patients werw randomly divided into group A and B （ each group of 20 cases ）. Local anesthetics were 0.5% levobupivacaine hydrochloride 20 ml, Group A not land with dezocinc, Group B in the local anesthetics with dezocine 0.05 mg/ kg. The time of anesthesia onset, the time to completely anesthesia, duration of anesthesia and postoperative VAS were recorded. Untoward effect ： to observe the phenomenon of the postoperative observation 36 h headache, dizziness, nausea, vomiting, itchy skin, respiratory inhibition, urinary retention and other adverse reaction. Results The duration of anesthesia in group A was sig- nificantly shorter than that in group B（ P 〈 0.05 ）. VAS scores at 3,6and 9 h postoperatively were lower than those in group A （P 〈0.05 ）. A group of the 10 cases should be injected Dolantin,but no one in group B. All of the patients the perioperative and postoperative all did not see respiratory inhibition, itchy skin, urinary retention complication. Conclusion Levobupivacaine hydrochloride together with dezocine on brachial block can prolong the anesthesia time and fewer side effects.
