自我效能感对社区老年人慢性疼痛调适的中介效应

摘　　要：目的分析慢性疼痛自我效能感在心理状态和应对方式对老年慢性疼痛调适影响过程中的中介作用,完善自我效能感对老年慢性疼痛调适影响的理论依据。方法采用一般人口学资料调查表,简明疼痛量表,汉化版慢性疼痛自我效能感量表,激惹、抑郁和焦虑自评量表,医学应对问卷对192例老年慢性疼痛患者进行调查。结果通过中介效应分析,疼痛管理自我效能感在抑郁和屈服型应对方式对疼痛调适影响的过程中产生了中介作用,中介效应依次为0.041和0.060,分别占总效应的40.2%和63.8%(均P<0.05);躯体功能自我效能感在屈服型应对方式对疼痛调适影响的过程中产生了中介作用,中介效应为0.097,占总效应的74.0%(P<0.05);症状应对自我效能感在抑郁和屈服型应对方式对疼痛调适影响的过程中产生了中介作用,中介效应依次为0.002和0.002,分别占总效应的3.2%和5.6%(均P<0.05)。结论慢性疼痛自我效能感在抑郁和屈服型应对方式对疼痛调适影响的过程中起到了中介作用,其中疼痛管理自我效能感和躯体功能自我效能感的中介效应较大。Objective To explore how self-efficacy mediates the relationship among mental state, coping styles and adjustment to chronic pain in elderly community residents. Methods The demographic questionnaire, the Brief Pain Inventory, the Chronic Pain Self-efficacy Scale of Chinese version, the Irritability, Depression and Anxiety Scale, the Medical Coping Modes Questionnaire were used to investigate 192 patients in chronic pain. Results Pain management self-efficacy mediated the relations both between de- pression, between resignation, and adjustment to chronic pain, with the mediation effects being 0. 041 and 0. 060 respectively, ac- counting for 40.2 % and 63.8 % of total effect（P〈0.05 for all） ; physical function self-efficacy mediated the relation between resig- nation and adjustment to chronic pain, with the mediation effects being 0. 097, and explaining 74.0% of total effect（P〈0.05 for both）; symptoms coping self-efficacy mediated the relations both between depression, between resignation, and adjustment to chronic pain, with the mediation effects being 0. 002 and 0. 002, and explaining 3.2% and 5.6% of total effect respectively（P〈 0.05）. Conclusion Chronic pain self-efficacy mediates the relation between depression, between resignation, and adjustment to pain; pain management self-efficacy and physical function self-efficacy play more important roles in the total effects..
