摘 要:目的探讨清胰解毒颗粒治疗重症急性胰腺炎(SAP)的可能机制。方法 48只SD大鼠随机分对照组、模型组、低剂量组、高剂量组、奥曲肽+低剂量组、奥曲肽组,每组各8只,采用L-盐酸精氨酸腹腔注射建立SAP大鼠模型。造模成功后对照组、模型组给予生理盐水7.5ml/kg灌胃,低剂量组、高剂量组分别给予清胰解毒颗粒11.3、56.5g/kg灌胃,奥曲肽组给予奥曲肽0.5μg/100g四肢部皮下注射,奥曲肽+低剂量组给予清胰解毒颗粒低剂量灌胃和奥曲肽皮下注射。每6小时1次,各组共给药4次后取胰腺组织进行病理学观察,并测定各组大鼠血清内毒素、白介素-6(IL-6)水平。结果对照组大鼠胰腺组织结构正常,细胞结构完整。模型组大鼠的胰腺组织可见胰腺正常结构消失,腺体呈片状坏死,大量淋巴细胞和中性粒细胞浸润,被膜下及间质水肿明显。奥曲肽组、奥曲肽+低剂量组、高剂量组、低剂量组大鼠胰腺小叶组织多为局灶或小片状坏死,出血、坏死较轻。与对照组比较,模型组大鼠血清内毒素及IL-6含量明显升高(P<0.01);与模型组比较,各给药组大鼠血清内毒素及IL-6含量明显降低(P<0.01);奥曲肽+低剂量组大鼠血清内毒素及IL-6含量较低剂量组、高剂量组和奥曲肽组降低明显(P<0.05或P<0.01)。结论清胰解毒颗粒治疗SAP机制可能为降低SAP模型大鼠IL-6和内毒素的水平,从而减轻炎症损伤,清胰解毒颗粒联合奥曲肽效果更好。Objective To study the mechanism of Pancreas-Clearing Toxin-Resolving Granules on severe acute pancreatitis (SAP).Methods Forty-eighty SD rats were randomized into control group,model group,low-dose group,high-dose group, low-dose+Octreotide group and Octreotide group,with 8 in each.The SAP rat models were made with intraperitoneal injection of L-arginine hydrochloride.The control group and model group was gavaged with 7.5ml/kg of normal saline respectively after modeling.Drug administration was applied once every six hours,and the low-dose group and high-dose group was gavaged with 11.3g/kg and 56.5g/kg of Pancreas-Clearing Toxin-Resolving Granules respectively.The Octreotide group was given a subcutaneous injection of 0.5μg/100g Octreotide into the four limbs.The low-dose+Octreotide group was gavaged with low-dose of Pancreas-Clearing Toxin-Resolving Granules and injected with Octreotide subcutaneously.Drug administration was applied once every six hours,and the pancreatic tissue was drawn for histopathological observation after four times of administration. The contents of endotoxin and interleukin-6(IL-6)were detected.Results The pancreatic tissue structure was closely and the cell structure was completed in the control group.The normal structure of the pancreas was disappeared,the gland was necrosis,a large number of lymphocytes and neutrophils were infiltrative and subcapsular and interstitial was edema in the model group.The pancreatic lobules were focal necrosis or small necrosis in the low-dose group,high-dose group,low-dose+Octreotide group and Octreotide group.Comparing with the control group,the contents of endotoxin and IL-6were significantly increased in the model group(P0.01).Comparing with the model group,the contents of endotoxin and IL-6were significantly increased in the administrated groups(P0.01),especially in the low-dose+Octreotide group(P0.05or P0.01).
