交叉克氏针内固定与保守治疗MasonⅡ型桡骨小头骨折比较

摘　　要：目的:研究交叉克氏针内固定与保守治疗MasonⅡ型桡骨小头骨折的临床疗效,为临床治疗提供参考。方法:对43例MasonⅡ型桡骨小头骨折的治疗进行回顾分析,其中22例行交叉克氏针内固定治疗,21行保守治疗。按Broberg-Morrey肘关节功能评分标准对两组肘关节功能进行评分。结果:全部病例获得随访,随访时间10～30个月,平均20个月。肘关节功能评分:交叉克氏针内固定治疗组:平均96.7分;其中极好16例,好5例,一般1例,总优良率为95.5%;保守治疗组:平均84.5分;其中极好12例,好6例,一般3例,总优良率为85.7%.交叉克氏针内固定治疗组明显优于保守治疗组,两组比较差异有统计学意义(P<0.05)。结论:交叉克氏针内固定治疗MasonⅡ型桡骨小头骨折在关节活动、肌力和功能恢复方面较保守治疗有明显优势。Objective：To compare the clinical results of crossing Kirschner wire internal fixation and conservative therapy in treatment of Mason type-Ⅱ radial head fracture, and provide data for the clinical treatment. Methods.. Retrospectively a- nalysis of 43 patients with Mason type- Ⅱ fracture of radial head was conducted. 22 cases were treated by open reduction and crossing Kirschner wire fixation, and 21 cases were treated by conservative therapy. The results were evaluated by the Broberg and Morrey functional rating score. Results.. All the patients were followed up for an average of 20 months （ranged from 10 to 30 months）. According to the Brobrg and Morrey functional elbow index, the averaged score in Kirschner wire fixation group was 96.7points and the outcome was excellent in 16 patients, good in 5, and the excellent and good rate was 95.5%. The averaged score in conservative therapy group was 84.5points and the outcome was excellent in 12 patients, good in 6, the excellent and good rate was 85.7%. The clinical efficacy of Kirschner wire fixation group was better than that of conservative therapy group （P〈0.05）, and the difference has statistically significance. Conclusion：The patients with Mason type- Ⅱ fractures of radial head treated with crossing Kirschner wire internal fixation have satisfactory joint motion, with greater strength and better function than those undergoing conservative therapy.
