摘 要:目的探讨高血压合并心房颤动患者颈动脉血流动力学变化及其对粥样斑块形成的影响。方法收集70例原发性高血压患者(高血压组)、67例高血压合并心房颤动患者(高血压合并心房颤动组)及60名健康成年人(对照组)行颈动脉彩色多普勒超声检查,测量并计算粥样斑块总面积(TPA)、颈动脉内径(CAD)、内中膜厚度(IMT)、颈动脉最大剪切率(SRmax)、颈动脉最小剪切率(SRmin)、搏动指数(PI)及阻力指数(RI),并进行统计学分析。结果与对照组比较,高血压组及高血压合并心房颤动组的TPA、CAD及IMT均增加,而SRmax及SRmin下降,差异均有统计学意义(P均<0.05);与高血压组比较,高血压合并心房颤动组患者TPA增加,而SRmax及SRmin下降,差异均有统计学意义(P均<0.05)。结论高血压合并心房颤动患者颈动脉TPA增加、剪切率下降。Objective To explore the impact of carotid atherosclerosis formation and hemodynamic changing in hypertension patients with atrial fibrillation. Methods Seventy patients with hypertension (hypertension group), 67 patients with hypertension and atrial fibrillation (hypertension and atrial fibrillation group) and 60 healthy subjects (control group)underwent color Doppler ultrasonography of carotid artery. The total plaque area (TPA), carotid artery diameter (CAD), intima-media thickness (IMT), carotid artery maximum shear rate (SRmax), minimum shear rate (SRmin), pulsatility index (PI) and resistance index(RI) were measured and statistical analyzed. Results TPA, CAD, IMT in hypertension group and hypertension with atrial fibrillation group obviously increased, but SRmax, SRmin obviously decreased compared with those of control group, and the differences were statistically significant (all P〈0.05). TPA in hypertension with atrial fibrillation group significantly increased, but SRmax, SRmin significantly descended compared with those of hypertension group (all P〈0.05). Conclusion Carotid TPA increase and SRmax, SRmin decrease in patients with hypertension and atrial fibrillation.
