700例改良内翻式大隐静脉剥脱术治疗单纯性大隐静脉曲张

摘　　要：目的：探讨改良内翻剥脱加点式抽剥术治疗下肢静脉曲张的手术方法及疗效a方法：对2004年10月～2010年10月问，应用该手术方法治疗的700例（752条下肢）下肢静脉曲张患者的临床资料进行回顾性研究。观察其手术时间、术后住院时间、并发症、手术效果及对切口外观满意度、成本一效益比等。结果：本组肢体行大隐静脉高位结扎内翻剥脱＋曲张静脉点式抽剥，手术时间平均60分钟；手术后平均1．5天出院。所有患者术后下肢静脉曲张消失，临床症状缓解。隐神经损伤（小腿内踝区皮肤麻木感）20例（2．6％）。无严重并发症发生。580例患者获得随访，平均随访24（1—48）个月，无明确的下肢静脉曲张复发；隐神经损伤的患者在术后3～8个月恢复。无下肢深静脉血栓等严重的并发症发生，成本一效益比满意。结论：改良内翻剥脱加点式抽剥术治疗下肢静脉曲张的手术方法是外科方法中简单、微创、有效、成本一效益比满意的治疗方法。Object ive：To evaluate the improved invaginated vein striping and minimally phlebectomy in the treatment of varicose vein. Methods .. From Oct. 2004 to Oct. 2010,700 patients（752 limbs） with varicose veins received improved in- vaginated vein striping and minimally phlebectomy, and the clinical data were analyzed retrospectively. Results：Satisfied surgical results with varicose veins reflux disappeared were obtained in all cases. The average operative time and the aver- age hospitalization after operation were 60 minutes and 1.5 days, respectively. Saphenous nerve damage occurred in20 （2. 6 % ）cases. Moreover, 580 patients were followed up with a mean time of 24 （1 - 48）months, no remained varicose veins, and cases witll saphenous nerve damage recovered in 3--8 months postoperatively. No serious complications such as deep venous thrombosis occurred and the satisfied cosmetic effects and cost--effect ratio achieved. Conclusion：Treatment of varicose veins of the lower limbs by improved invaginated vein striping and minimally phlebectomy has the benefit of mini --invasion,fast recovery,and good curative effect.
