成年女性血型对氯胺酮麻醉时精神症状的影响

摘　　要：目的：探讨成年女性血型对氯胺酮麻醉时精神症状和血流动力学的影响。方法；89例拟行单侧乳腺肿块切除术的患者，根据血型分为四组：A组（A血型）、B组（B血型）、O组（O血型）、AB组（AB血型）。所有患者缓慢静推氯胺酮2mg／kg，术中麻醉偏浅追加氯胺酮lmg／kg。出现精神症状静注咪唑安定2mg／次。监测所有患者基础血压、心率和Sp02以及消毒时、意识消失时、注药后5、10、15min、手术结束时和完全苏醒时的血压、心率和Sp02。记录术中及苏醒期出现精神症状的病例数。结果：A和AB组术中和苏醒期出现阳性症状的比例（48％、50％）和阴性症状的比例（18％、17％）明显高于B组（40％、5％）和O组（25％、12．5％）（P〈0．05）；AB组患者意识消失时、注药后5和10min时的收缩压明显高于其他组（P〈0．05）；A和AB组氯胺酮和咪唑安定的用量明显多于B组和0组（P〈0．05）。结：成年女性血型对氟胺酮时精神症状的表现有明显影响。Objective To investigate the effects of adult female＇s blood group on mental symptoms and hemodynamics during period of ketamine anesthesia. Methods 89 patients scheduled for unilateral breast mass excision divided into four groups by blood types ：group A, group B, group O, group AB . All patients were anesthetized by 2mg/kg ketamine, lmg/kg ketamine was added when inadequate analgesia. Midazolam 2rag was injected when mental symptoms appear. The blood pressure, heart rate and SpO2 were measured at the time of disinfection and loss of consciousness, 5, 10 and 15min after injecting ketamine,the end of surgery,the time of fully revival. The number of cases with mental symptoms during period of ketamine anesthesia and awaking are recorded. Results The proportion of patients in group A and AB with posi- tive mental symptoms （48 ~, 50 ~ ） and negative mental symptoms （ 18 %, 17 ~ ） during period of ketamine anesthesia and a- waking is higher than in group B（40 ~, 5 ~ ） and O（5 ~, 12.5 ~ ） （P 〈~0.05） ;The consumptions of ketamine and midazo- lam in group A and B are more than in group B and O（P d0.05） ;The SBP of patients in group AB at the time of loss of consciousness, 5and 10rain after injecting ketamine are higher than in group A,B and O（P ＂（0.05）. Conclusion Adult fe- male＇s blood group has a significant effect on mental symptoms during period of ketamine anesthesia.
