摘 要:目的探讨多囊卵巢综合征(PCOS)患者月经周期紊乱与内分泌代谢异常的相关性。方法 PCOS患者186例,按月经周期分成3组:月经规则组30例,月经稀发组102例,闭经组54例,正常对照组25例。分别测定身高、体质量、血压、空腹血糖、空腹胰岛素、血脂、性激素(卵泡刺激素、黄体生成素、睾酮),利用稳态胰岛素评估模型计算胰岛素抵抗指数(HOMA-IR),同时测定患者体质量指数(BMI),并在B超下测定双卵巢的体积及卵泡数。结果正常对照组、正常月经周期PCOS患者代谢参数(BMI、空腹血糖、空腹胰岛素、高密度脂蛋白、HOMA-IR)明显好于月经稀发和闭经的PCOS患者(P<0.05)。性激素、卵巢体积及卵泡数在正常对照组及PCOS各组间存在明显差异(P<0.05)。结论 PCOS患者月经周期紊乱与内分泌代谢异常相关,月经周期紊乱可能是衡量内分泌代谢失调的指标之一。【Objective】 The aim of this study was to evaluate possible correlations between the degree of cycle irregularity and the grade of endocrine and metabolic abnormalities in polycystic ovarian syndrome(PCOS) patients.【Methods】 A cross-sectional study was carried out.186 PCOS patients and 25 controls were involved in this study.186 with PCOS were assigned to three groups: 30 PCOS patients with regular menstrual cycles,102 PCOS patients with oligomenorrhea and 54 PCOS patients with amenorrhea.Height,weight,blood pressure,fasting serum insulin,blood glucose,triglyceride,high density lipoprotein and hormonal parameters were examined and homeostasis model assessment for resistant(HOMA-IR) index was calculated.Body mass index(BMI) of all the patients were calculated and ovarian volume was calculated by ultrasound examination.【Results】 Controls and normocyclic patiens demonstrated significantly better metabolic parameters(BMI,high density lipoprotein,fasting serum insulin,fasting blood glucose and HOMA-IR) than patiens with oligo/amenorrhea.Hormonal parameters and ovarian volume and follicle were significantly different between controls and patients with different menstrual patterns.【Conclusion】 The degree of cycle irregularity had a significant relationship with endocrine and metabolic abnormalities.And the degree of cycle irregularity may be a simple and useful parameter to predict PCOS patients with a risk of endocrine and metabolic disorders.
