小切口离断式肾盂成形术与传统离断式肾盂成形术的对比研究

摘　　要：目的探讨小切口离断式肾盂成形术在治疗肾盂输尿管连接部梗阻的疗效及可行性,并与传统离断式肾盂成形术的各项数据进行对比研究。方法回顾性分析2006年5月～2011年4月该科行开放手术治疗肾盂输尿管连接部狭窄的患者54例。其中行小切口离断式肾盂成形术24例,传统离断式肾盂成形术30例。比较其手术时间、估计出血量、术后胃肠功能恢复时间、术后住院时间的差别并进行统计学分析。结果所有手术均获得成功,术中及术后未出现严重的并发症。小切口离断式肾盂成形术组的手术时间为(97.50±23.17)min,术中估计出血量(66.67±24.08)mL,术后胃肠功能恢复时间(2.17±0.82)d,术后住院天数(7.96±1.60)d。传统离断式肾盂成形术组的的手术时间为(113.17±28.30)min,术中估计出血量(141.67±60.29)mL,术后胃肠功能恢复时间(2.67±0.71)d,术后住院天数(9.63±2.71)d。小切口离断式肾盂成形术组在手术时间、术中估计出血量、术后胃肠功能恢复时间、术后住院天数的指标上均低于传统离断式肾盂成形术组。结论小切口离断式肾盂成形术与传统离断式肾盂成形术相比,在安全性和可行性上无明显差别。小切口离断式肾盂成形术同样获得满意的手术视野,具有出血少、创伤小、痛苦少、术后恢复快等优点,适合用来替代传统离断式肾盂成形术,是一种较为理想的微创性肾盂成形术。【Objective】 To compare the various data between small incision open dismembered Pyeloplasty（ODP） and conventional open dismembered Pyeloplasty doing at the same time,and to discuss the feasibility and healing effect of the small incision ODP in the treatment of ureteropelvic junction obstruction（UPJO）.【Methods】 Clinical data of 54 cases of UJPO treated with open surgery in our department from 2006 to 2011 were investigated retrospectively.Of the 54 Patients,24 underwent the operation of small incision ODP,30 were subjected with conventional ODP.In the study,we compared the clinical data between the 2 group including the operative time,intraoperative estimated blood loss,Postoperative intestinal function recovery,Postoperative hospital stay.【Results】 All of the operations were successfully.There were no severe Perioperative or Postoperative complications that could be related to the operations.In the small incision ODP group,the operative time,intraoperative estimated blood loss,Postoperative intestinal function recovery and Postoperative hospital stay were（97.50±23.17） min,（66.67±24.08） mL,（2.17±0.82） d,（7.96±1.60） d.And the data in the conventional ODP group were（113.17±28.30） min,（141.67±60.29） mL,（2.67±0.71） d,（9.63±2.71） d.The opera-tive time,intraoperative estimated blood loss,postoperative intestinal function recovery and postoperative hospital stay in the small incision ODP group were less than the conventional ODP group（P〈0.05）.【Conclusions】 Compare with the conventional ODP,there has no obviously difference on the safety and feasibility.Small incision ODP has several advantages over conventional ODP,namely less blood loss,small wound,less Pain and more rapid recovery and so on.As a kind of minimal invasive Pyeloplasty,we believe that it is suitable to replace the conventional ODP.
