摘 要:目的初步探讨益生菌在预防危重患者腹泻方面的作用。方法选取2009年5月至2010年9月本院呼吸内科重症监护室需行肠内营养的患者,分为常规喂养组(对照组,n=38)和加用益生菌组(研究组,n=42)。对照组给予肠内营养液(能全力)鼻饲,研究组在常规喂养的基础上加用益生菌酸奶鼻饲。比较两组腹泻发生率、发生次数及腹泻相关因素、住院时间等指标。结果研究组第5、6、7天腹泻例数分别为2、3、3例,较对照组的9、8、7例减少(P〈0.05);便培养中白色念球菌与真菌孢子发生率分别为3例和2例,低于对照组的9例和7例(P〈0.05);住院时间[(13.17±3.79)d比(15.18±4.39)d,P=0.031]、住院费用[(6307±2358)元比(8253±3186)元,P=0.002]、家属满意度均优于对照组。结论益生菌对于预防危重患者肠内营养相关性腹泻有一定的效果。Objective To explore the role of probiotics in the prevention of diarrhea in critically ill pa- tients. Methods The patients receiving enteral nutrition from May 2009 to September 2010 in the Respiratory In- tensive Care Unit were divided into routine feeding group (control group, n = 38) and probiotic yogurt feeding group (yogurt group, n = 42). The control group accepted routine enteral nutrition liquid ( Nutrison), while the yogurt group accepted conventional feeding plus probiotic yogurt. The incidence of diarrhea, length of hospital stay, and other indicators were compared between these two groups. Results The incidence of diarrhea was lower in the yogurt group on the 5th, 6th, and 7th days (P〈0. 05). The cases of positive stool bacterial and fungal culture results in the yogurt group were less than those in the control group (P 〈 0. 05 ). In addition, the hospital stay was shorter [ (13. 17 ± 3.79) d vs. (15. 18 ± 4. 39) d, P = 0. 031 ], the cost was lower [ (6307 ± 2358) yuan vs. (8253 ± 3186) yuan, P =0. 002], and the satisfaction of the patients were higher in the yogurt group. Conclusion Probiot- ies may reduce the occurrence of diarrhea associated with enteral nutrition in critically ill patients.
