自体真皮移植矫正下眼睑凹陷的临床应用

摘　　要：目的探讨应用自体真皮填充下睑凹陷,并同时对部分伴发的下睑外翻予以纠正的手术体会和临床应用。方法对我科2010年1月至2011年6月收治的12例(13眼)下睑凹陷的患者予以自体真皮填充,并对其中7例(7眼)伴发轻度睑外翻的患者予以矫正。结果对12例(13眼)患者术后6～12个月进行了随访,11例(12眼)术后恢复好,眼睑凹陷填充满意,无外翻,无需再次手术治疗;1例(1眼)凹陷填充满意,但因患者术后没有规律佩戴支具及用瘢痕药,下睑出现轻度外翻,患者认为效果尚可,未再次手术。结论应用自体真皮移植矫正下眼睑凹陷,效果良好,无排异性,取材丰富,成活率高,吸收率低,一次手术即可达到满意效果,并可同时进行轻度下睑外翻的矫正,值得推广。Objective To summarize the operation experience and clinical application of autologous dermal graft to correct lower eyelid depression and repair concomitant lower eyelid ectropion. Methods Thir- teen lower eyelid depression in 12 patients were treated with autologous dermal transplantation in our department from January 2010 to June 2011. And 7 lower eyelid ectropion from 7 patients were repaired at the same time. Results These 13 eyes of 12 patients were followed up for 6 to 12 months. Twelve eyes of 11 patients got the satisfying outcome with eyelid depression filling and no ectropion was seen. Only 1 eye appeared mild ectropi- on, but the patient thought it was fair effective, so no more operation was carried out. Conclusion Applica- tion of autologous dermal graft to correct lower eyelid depression is an effective approach with material-rich, no immune response, high survival rate and low absorption rate. Concomitant lower eyelid ectropion can be re- paired at the same time. So this approach is worthy of being popularized.
