手指末节远段复合性外伤综合修复探讨

摘　　要：【摘要】目的探讨手指末节远段外伤后综合修复手术技巧。方法根据患指损伤累及的解剖结构，分别应用皮瓣等方法修复创面，应用骨片钉等方法治疗末节骨折，结合甲床移植及甲床扩大手术恢复甲外形。结果本组共17例患者（19指），术后从指腹外形、感觉等方面进行随访3—5个月，患指皮瓣成活，指骨骨性愈合，患指感觉及外形满意。结论对于手指末节远段损伤的修复既应进行针对性治疗，又需进行整合考虑并综合修复，以得到最佳的治疗效果。Abstract： Objective To discuss the comprehensive reparative surgical technique for the treatment of traumatic distal finger injury. Methods According to the different anatomical structures involved, a compre- hensive repair containing respectively matching reconstructive technique was conducted： skin flap for wound re- covery, fragment pin for digital phalangeal fracture and nail bed lengthening or free grafting for the restoration of a normal fingertip profile. Totally 17 patients with 19 injured fingers were included in this study and cases were followed up and assessed by criteria of pulp outline, sensory, movement and et cetera. Results All flaps sur- vived while fractures achieved bony union, follow-up results of the above criteria were satisfactory. Conclus ion The repair for combined traumatic distal finger injury needs not only targeted surgical procedure based on injured anatomical units but also an overall consideration by plastic surgeons, therefore, the application of com- prehensive reparative surgical technique might obtain optimum effects.
