摘 要:目的观察Ⅱ型糖尿病对扩展高频测听及畸变产物耳声发射(distortion product otoacousticemission,DPOAE)的影响。方法选择自觉听力正常32例和异常的糖尿病患者28例及正常对照组30例进行扩展高频测听及DPOAE的检测,对其结果进行对照比较。结果同一年龄段糖尿病组扩展高频测听阈值较对照组升高,17~40岁患者12kHz及以上频率闽值增高有显著意义(P<0.05),41~55岁年龄组10~14kHz频率闽值增高有显著意义(P<0.05),糖尿病患者的DPOAE幅值在1000~8000Hz出现下降,其中在2000,4000,8000Hz均有统计学意义。结论 DPOAE和扩展高频测听可以在早期检测出糖尿病患者的听力损害,临床上可作为Ⅱ型糖尿病患者耳蜗功能早期变化的有效监测手段。Objective To observe the efects of diabetes mellitus on amplitude of distortion product otoacoustic emissions(DPOAE). Methods forty diabetic patients with normal heating and forty with unnormal heating and forty healthy people as control were collected in this study.The ampiltude of extended high-frequency audiometry and DPOAE was measured and the data were calculated in the three groups. Restdts At each frequency tested, the DM subjects had higher hearing thresholds than the control subjects, however, only the differences between the two groups were significant above 12 kHz at age from 20 to 40 years old and above 10 kHz at age from 41 to 55 years old(P〈0.05) .The amplitude of DPOAE in diabetic patients decerased in 1000-8000 Hz and had statistical significant difference at 2000,4000and 8000Hz compared with the contorl group.Conclusion DPOAE and Extended high-frequency audiometry could be used as means of the early detection of heating loss in patients with diabetes mellitus, so they can be used as effectives index for detecting early changes of cochlear function in patients with type 2 diabetes.
