超声弹性成像与多普勒血流显像对乳腺实性病灶鉴别价值的对比研究

摘　　要：目的探讨彩色多普勒血流显像(CDFI)与实性弹性组织成像(URTE)对乳腺实性病灶良恶性鉴别诊断的价值。方法对经病理证实的260例患者373个乳腺实性病灶,在高频超声检出病灶后,对病灶进行CDFI与URTE检查前瞻性研究。病灶的血流分级采用Adler半定量法;URTE评分采用改良5分法。结果 CDFI对乳腺病灶良恶性鉴别诊断的敏感性85.7%、特异性74.4%、准确性77.0%、阳性预测值49.3%、阴性预测值94.7%、约登指数0.6。而URTE的敏感性83.3%、特异性93.8%、准确性91.4%、阳性预测值79.6%、阴性预测值95.1%、约登指数0.8。结论 URTE在诊断特异性、准确性和阳性预测值方面较CDFI有明显的优势,反映诊断试验真实性的综合指标约登指数也优于CDFI。URTE应和CDFI一样作为临床鉴别乳腺实性病灶良恶性的重要影像技术手段。Objective To analyze the value of color Doppler flow imag!ng （CDFI） and ultrasonic real-time tissue elastography （URTE） to mammary gland real venereal benign and malignant focal differential diagnosis. Methods Two hundred and sixty cases of pathologically proven of 373 breast real venereal kitchen, in the high frequency ultrasound detection lesions after, were checked by URTE and CDFI prospectively. Blood flow grading of lesion was tested by the method of Adler half quantitative. Results In the diagnosis of CDFI to benign and malignant lesions, the senitivity was 85.7%, the specificity was 74.4%, the accuracy was 77.0%, the prediction of positive was 49.3%, the prediction of negative was 94.7%, the youden index was 0.6. In the diagnosis of URTE, the sensitivity was 83.3%, the specificity was 93.8%, the accuracy was 91.4%, the prediction of positive was 79.6%, the prediction of negative was 95.1%, the youden index was 0.8. Conclusion URTE in diagnosis of specificity, accuracy and positive has obvious advantages compared to CDFI, index （YI） reflected diagnostic tests the authenticity of the comprehensive index, is better than CDFI. And URTE as well as CDFI, should be identified as the clinical breast sohd mass of benign and malignant video technology means important.
