摘 要:目的 探讨螺旋CT门静脉造影 (CTP)对门静脉系统的显示能力 ,评价其在该系统的临床应用价值。方法 16 0例疑有肝脏疾病的患者进行了CTP检查 ,其中经手术治疗者 47例 ,常规血管造影者 12例 ,以此为对照 ,对肝脏肿瘤定位及门静脉癌栓、门脉高压侧支循环的显示情况和不同疾病的CTP表现做一分析。结果 CTP能清晰成像门静脉系统 ,精确定位肝内病灶 (10 0 % ) ,CTP原始图像和多轴向投照容积重建 (MPVR)检出门静脉癌栓的准确率达 10 0 % ,并可检测门脉高压侧支循环的范围和估计严重程度 ,以及显示多种疾病。结论 螺旋CT门静脉造影是门静脉无创性检查的可靠方法 。Objective To evaluate the ability of spiral CT portography (CTP) to image the portal vein and its diagnostic value. Methods CTP was performed in 160 cases suspected of hepatic disease. 47 patients received surgical treatment and 12 with routine angiography. The demonstration of the location of hepatic lesions, the portal vein tumor thrombosis and the collateral circulation in portal hypertension, and the CTP findings in a variety of diseases were analyzed. Results The portal vein could be distinctly displayed by CTP. It could exactly locate the hepatic lesions (100%) and the accurate rate of finding the portal vein tumor thrombosis was 100% with CTP natural images and multi-projection volume reconstruction (MPVR), and it also could show the range of collateral circulation and estimate its degree. It could be used in a spectrum of abnormalities. Conclusion CTP is a noninvasive, reliable, and valuable method for the assessment of portal venous system.
