原发性高血压晨峰现象患者心率变异性和心律失常分析

摘　　要：目的探讨原发性高血压患者血压晨峰与心率变异性(Heart Rate Variability,HRV)和心律失常的特点。方法对83例高血压患者行24小时动态血压监测的同时行24小时动态心电图监测,其中血压晨峰高于55 mmHg 39例(晨峰组),非晨峰组44例。对两组HRV各参数和心律失常进行比较。结果晨峰组HRV各参数显著低于非晨峰组,心律失常发生率明显高于非晨峰组,差异有统计学意义(P<0.05)。结论原发性高血压患者出现晨峰现象提示自主神经功能受损,心律失常发生率明显增加,特别是房性心律失常发生率明显增加。Objective To study the characteristics of heart rate variability（ HRV） and arrhythmias in the morning blood pres- sure surge of patients with hypertension. Methods Eighty-three patients with primary hypertension were monitored by 24-h ambulatory blood pressure and 24-h ambulatory electrocardiogram. The patients with morning blood pressure surge ≥ 55 mmHg were designed as the morning blood pressure surge group （n = 39 ）and others as non-morning blood pressure surge group（ n = 44 ）. Parameters of HRV and arrhythmias incidence were compared between the two groups. Results The parameters of HRV including SDNN, SDANN, RMSSD and PNNS0 of morning blood pressure surge group were significantly lower than those of non-morning blood pressure surge group. The inci- dence rate of arrhythmias was increased in morning blood pressure surge group compared to non-morning blood pressure surge group. Conclusions The morning blood pressure surge in patients with primary hypertension indicate that the autonomic never function has been damaged. The incidence rate of arrhythmias, especially atrial arrhythmia, is increased.
