摘  要:目的探讨脊柱结核的高场强磁共振(MRI)表现。方法回顾性分析经临床确诊的47例脊椎结核患者的1.5T MRI征象。结果 47例患者中,椎体结核45例,包括颈椎结核3例、胸椎结核20例、腰椎结核10例、骶椎结核1例、胸腰椎结核8例、腰骶椎结核2例、胸腰骶椎结核1例;附件结核及椎体伴附件结核各1例,其中腰椎附件结核1例,腰椎椎体伴附件结核1例。脊柱后凸、侧弯28例,椎体变形43例,椎体终板破坏3例,椎体破坏及椎体骨髓水肿46例,椎间盘炎1例,椎间盘破坏43例,椎旁脓肿46例,椎管内异常改变40例。结论 MRI能准确反映脊柱结核各期的病理改变,特别是对早期病变有独特优势,在脊柱结核诊断及鉴别诊断方面有重要价值。Objective To explore the high-field MRI features of spinal tuberculosis(TB). Methods MRI imaging data of 47 cases with spinal tuberculosis confirmed by surgery and pathology were analyzed retrospectively. All patients were examined by using a 1.5T MRI scanner. Results It was found that 45 of 47 cases were suffered from vertebral body TB. In these patients 3 cases were cervical vertebra TB ,20 cases thoracic vertebra TB, 10 cases lumbar vertebra TB, 1 case sacral vertebra TB ,8 cases thoraco-lumbar verte- bra TB ,2 cases lumbo-sacral vertebra TB, and 1 case thoraco-lumbo-saeral vertebra TB. Furthermore, one case was found to be tuberculosis of lumbar accessory and one case was lumbar vertebral tuberculosis with attachment. By using MRI, kyphosis and scoliosis were found in 28 cases, vertebral deformity in 3 cases, vertebral endplate destruction in 3 cases, destruction of the vertebral body and vertebral bone marrow edema in 46 cases, discitis in one ease, disc damage in 43 cases, paravertebral abscess in 46 cases, and intraspinal anomalies changes in 40 cases. Conclusion MRI can correctly show pathological changes of the spinal tuberculosis in different stages, especially the early spinal tuberculosis. Thus, MRI is one of preferred imaging examination methods for diagnosis and differential diagno- sis of soinal tuberculosis.
