摘  要:目的调查耐药鲍曼不动杆菌β-内酰胺酶基因型流行情况,为医院合理使用抗菌药物提供参考依据。方法用聚合酶链反应(PCR)方法对7株鲍曼不动杆菌的8种相关β-内酰胺酶基因型(TEM、AmpC、VIM、IMP、PER、GES、VEB、SHV)进行检测,并通过琼脂凝胶电泳后进行成像验证。结果 7株鲍曼不动杆菌中blaTEM染色体检测阳性有7株、质粒阳性7株;blaAmpC染色体阳性3株、质粒阳性6株;blaPER染色体阳性4株;其余β-内酰胺酶基因未检出,其中有1株同时携带有3种耐药基因型。结论β-内酰胺类药物耐药基因以TEM型最为常见,其次为ampC型、PER型,其余型未见发现;同一株耐药菌可携带多种耐药基因。Objective To study the prevalence of genotypes of muhidrug resistance of Acinetobacter baumannii in this hospital and to pro- vide reference to rational use of antibacterials. Methods Among 7 strains of A. baumannii, 8 kinds of β laetam - resistant genotypes ( TEM, AmpC, VIM, IMP, PER, GES, VEB and SHV) were detected by polymerase chain reaction (PCR) assay, and they were verified by agarose gel electrophoresis. Results In these 7 strains, blaTEM was positive in chromosome and plasmid of 7 strains, blaAmpC was positive in chromosome of 3 strains and plasmid of 6 strains, blaPER was positive in chromosome of 4 strains, and others were negative. There was one strain carrying three genotypes. Conclusion In this hospital, blaTEM was the most important resistant genotype, and blaAmpC, and blaPER were the next, and no other types had been found. A β lactam - resistant strain can carry more than one resistant genotype.
