老年2型糖尿病合并轻度认知功能损害的随访研究

摘　　要：[目的]探讨老年2型糖尿病（T2DM）合并轻度认知功能损害（MCI）患者认知功能的变化特点。[方法]研究对象为患MCI的老年T2DM患者（MCI组，n=48）和认知功能正常的老年健康对照组（对照组，n=48）。采用蒙特利尔认知评估表（MoCA）北京版、简易精神状态量表（MMSE）和画钟测验（CDT）行神经心理评估，在初次确诊及随访一年后评价分析比较。[结果]一年后随访，MCI组MoCA量表总分、视空间与执行功能、语言、延迟回忆评分、MMSE量表总分和CDT得分与基线得分比较均有不同程度下降，其中MoCA量表总分、视空间与执行功能、延迟回忆评分随访后与基线间差异均有统计学意义（P〈0．05），且与对照组同期比较差异有显著性（P〈0．05）；对照组MoCA量表总分、视空间与执行功能、延迟回忆评分、MMSE量表总分和CDT得分与基线得分比较均有不同程度下降，但差异无统计学意义（P〉0．05）。[结论]延迟回忆和视空间与执行功能受损是老年T2DM合并MCI的早期特征性表现。[Objective]To observe and evaluate the changes of cognitive function of elderly type 2 diabetes mellitus（T2DM） with mild cognitive impairment（MCI）. [Methods]A total of 48 elderly T2DM patients with MCI（MCI group, n ~ 48） and 48 elderly healthy controls with normal cognition（control group, n = 48） were selected. Montreal cognitive assessment（MoCA）（Beijing edition）, mini mental state examination（MMSE）and clock drawing test（CDT） were used for neuropsychology assessment. All patients were evaluated and compared at initial definitive diagnosis and a year after follow up. [Results]Compared with baseline scores, the total score of MoCA, visuospatial skill, executive function, language, delayed recall score, total score of MMSE and CDT score in MCI group after a year of follow up decreased at difference degrees, and there were signifi- cant differences （ P〈0.05）. Compared with baseline scores, the total score of MoCA, visuospatial skill, ex- ecutive function, delayed recall score, total score of MMSE and CDT score in control group decreased at different degrees, but there was no significant difference（ P 〉0.05）. [Conclusion]Delayed recall, visuospatial and executive impairment are the early characteristic manifestations of elderly T2DM with MCI.
