围手术麻醉期不同体温保护措施的比较

摘　　要：目的观察全身麻醉下不同体温保护方法对患者术中中心体温的影响。方法选取择期在全身麻醉下行下腹部手术患者90例,分为3组,每组30例,即常规保温组,室温控制组(常规保温+控制室温)及综合保温组(综合保温+控制室温)。取双侧耳鼓膜温度平均值为中心温度,记录患者入室后、麻醉后即刻、麻醉后15、30、45、60、75、90、105、120 min及术毕各时间点体温变化。结果术中常规保温组患者体温在45 min后、室温控制组在75 min后明显下降,与入室体温比较差异有统计学意义(P<0.05);综合保温组在各时点与入室体温比较差异无统计学意义。常规保温组体温在45、60 min与室温控制组同时点差异有统计学意义(P<0.05),常规保温组在45 min后、室温控制组在75 min后与综合保温组同时点差异有统计学意义(P<0.05)。结论术中积极的体温保护措施可有效地防止患者的体温丢失,提升室温对预防短小手术患者体温下降有较好效果。Objective To investigate the effects of different body temperature protections on central body temperature in patients undergoing general anesthesia. Methods 90 patients, scheduled for operation on abdominal region, were divided into three groups： regular preservation group （ n = 30） , room - temperature control group （ n = 30） and synthetic preservation group （ n = 30 ）. The tympanic temperature which was taken as central body temperature was recorded before induction, at 15,30, 45, 60, 75, 90, 105 and 120 min after induction and the end of operation. Results Compared with base temperature, the central body temperature descend after 45 rain in common preservation group and after 75 min in room - temperature control group （ P 〈 0.05 ）. There was no statistical significance of central body temperature in synthetic preservation group. There was significant difference in the regular preservation group and the room - temperature control group at 45 min and 60 min （ P 〈 0.05 ）. Compared with synthetic preservation group, the temperature descend remarkably after 45 rain in common preservation group and after 75 min in room - temperature control group （ P 〈 0.05 ）. Conclusions It is effective to protect body temperature from hypothermia with active preservations intraoperatively. Promotion of room temperature can prevent temperature descend in minor surgeries.
