群组干预对社区高血压患者治疗依从性的影响

摘　　要：目的探讨群组干预对社区高血压患者治疗依从性的影响。方法选择已确诊且在社区已建健康档案的高血压患者1183例作为研究对象,随机分为实验组(n=592)和对照组(n=591),实验组实施群组干预活动,对照组实施常规三级预防管理,评价两组干预前后的相关指标。结果实验组干预后高血压知识得分、知晓率提高程度均高于干预前和对照组干预后(P<0.01);实验组干预后药物治疗(61.0%)、体育锻炼(53.2%)、饮食调理(82.9%)、情绪调节(90.7%)依从性均较干预前(分别为46.3%、34.6%、72.8%和75.5%)明显提高,且药物治疗、体育锻炼、情绪调节依从性均高于对照组干预后(P<0.05);两组干预前后患者的不吸烟、不饮酒依从性差异无统计学意义(P>0.05)。结论群组干预能明显提高社区高血压患者治疗依从性,患者对干预的依从性也较好,可作为社区高血压管理的补充。Objective To investigate the influence of group intervention on treatment compliance of hypertensive patients in community. Methods A total of 1183 cases with hypertension were selected, who had been diagnosed and had built a health profile in community. They were randomly divided into experimental group （ n = 592 ） and control group （ n = 591 ）, and the experimental group were received group intervention, the control group were received conventional three level prevention management. Before and after intervention, related indicators between the two groups were compared and evaluated. Results After intervention, hypertension knowledge score and awareness rate of experimental group were in- creased significantly than before intervention and than those of control group （ P 〈 0. 01 ）. After intervention, the drug treatment compliance, physical exercise compliance, diet conditioning compliance, emotional regulation compliance of ex- perimental group （61.0% , 53.2% , 82. 9% and 90.7% ） were all improved better than before intervention （46. 3% , 34. 6% , 72. 8% and 75.5% ） , and better than those of control group, the differences were statistically significant （P 〈 0. 01 ）. No significant difference was found in the two groups about ＂no smoking and no drinking＂ compliance （P 〉 0. 05 ）. Conclusion Group intervention can be used to improve treatment compliance of hypertensive patients in community and used as a supplement to the hypertension management in community for good patient intervention compliance.
