摘　　要：目的:探讨基本药物制度对湖北省基层医疗卫生机构合理用药的影响。方法:随机抽取基层医疗卫生机构门诊处方2000张和住院病人2000名,并采用统计学方法比较基本药物制度实施前后住院病人使用抗生素、抗生素联用比例,以及注射处方使用比例和激素处方使用比例,了解平均每方费用等指标的差异。结果:2010-2011年实施基本药物制度后,住院病人使用抗生素比例从70.50%下降为65.85%,门诊抗生素联用处方比例从37.00%下降到32.50%,门诊注射处方比例从62.65%下降到58.25%,门诊平均每方费用从44.32元下降到35.06元,差异都有统计学意义。结论:基本药物制度对基层医疗卫生机构合理用药有促进作用,但基层仍然存在抗生素、注射剂滥用的现象,提出基层医疗机构应加强对抗生素和注射剂的用药监管力度,以提高合理用药率,减少抗生素、注射剂滥用问题。Objective： To explore the impact on rational drug use of community health care centers with the implementation of primary Health Care Institutions of Hubei province. Methods： 2000 prescriptions and 2000 patients of 20 primary Health Care Institutions were randomly selected. Before and after the implementation of Essential Medicine System, the differences of the indicators as the proportion of the hospitalizations with antibiotics, the proportion of the prescription of antimicrobial combinations, injection and hormones, and the average amount per prescription were compared with statistical methods. Results： From 2010 to 2011, the proportion of the hospitalizations with antibiotics deceased from 70.50% to 65.85%, the proportion of prescriptions with two or more antimicrobial drugs deceased from 37.00% to 32.50%, the proportion of the prescription of injection deceased from 62.65% to 58.25%, the average amount per prescription deceased from ~44.32to^35.06, and the differences were statistically significant. Conclusion： The Essential Medicine System can play the role of promoting the rational use of drugs, hut its role is limited. It＇s necessary to change the doctors and patients attitudes and enhance the training and regulation about the rational drug use.
