中性粒细胞CD64表达在细菌性肺炎中的诊断价值

摘　　要：目的探讨中性粒细胞CD64表达在细菌性肺炎的诊断和抗感染治疗中的作用。方法对细菌性肺炎患者31例、其它肺部疾病患者15例及健康对照组25例用流式细胞仪检测中性粒细胞CD64表达,并对11例细菌性肺炎患者抗生素治疗前后CD64表达进行了对比。结果细菌性肺炎组CD64表达水平[(4825.10±3855.75)PE/cell]明显高于其他肺部疾病组[(1262.60±712.41)PE/cell]和健康对照组[(1277.92±886.51)PE/cell],差异有统计学意义(P<0.05)。11例细菌性肺炎患者抗生素治疗后7天CD64表达水平[(1671.16±581.11)PE/cell]明显低于治疗第3天[(3450.82±2161.92)PE/cell]和治疗第1天[(5339.91±3898.16)PE/cell](P<0.05)。同时作ROC曲线,计算得出CD64表达的截断值为1354.5,敏感性和特异性分别为90.3%和73.3%。结论中性粒细胞CD64表达适用于细菌性肺炎的诊断及抗感染治疗的观察。Objective To explore the clinical significance of the determination of neutrophil surface CD64 in the early diagnosis of bacterial pneumonia and its anti-infection effect. Methods The experimental group included 31 patients with bacterial pneumonia, 15 patients were not infected and 30 healthy subjects as controls. Their levels of CD64 were detected by flow cytometry. Comparison in CD64 between pre-and post-treatment of antibiotics in 11 cases infectious patients were also made. Results The level of CD64 in the bacterial pneumonia group [（4 825.10±3 855.75） PE/cell] was significantly higher than that in the non-bacterial pneumonia group [（1 262.60 ± 712.41） PE/cell] and healthy control group [（1 277.92± 886.51） PE/cell] （P〈0.05）. The level of CD64 in 7th day of post-treatment [（1 671.16± 581.11） PE/cell] was lower than that in 3rd day of post-treatment [（3 450.82 ± 2161.92） PE/cell] and in the pre-treatment [（5 339.91 ±3 898.16） PE/cell] （P〈0.05） in 11 cases of infectious patients. According to the ROC curve, the best threshold of CD64 was 1 354.5, and its sensitivity and specificity were 90.3% and 73.3% respectively. Conclusions The determination of CD64 may assist in the early diagnosis of bacterial pneumonia and the observation of anti-infection effect for infection patients.
