山东省医疗欠费原因分析与对策研究

摘　　要：目的:描述和评价山东省医疗欠费情况,分析医疗欠费产生的原因,针对性的提出预防和减少医疗欠费问题的措施。方法:分别采用文献查阅和分层随机抽样方法,对山东省医疗欠费总体情况和106所医疗机构的医疗欠费情况进行调查和分析。结果:医保欠费、"三无"病人欠费、公费医疗欠费和医疗欠费发生后缺少专项补偿基金是当前山东省医疗欠费发生和存在的主要原因。结论:完善社会医疗保障体系,扩大医保覆盖范围;优化医保结算方式,提高医保费用结算效率;建立医疗救助制度,保障弱势群体医疗卫生需求;学习先进经验,稳步推进公费医疗改革;建立医疗欠费补偿基金,保障医疗机构的正常发展。Objective To describe and evaluate status of medical treatment arrearages in medical institutions of Shandong Province, to analyze the reasons of medical treatment arrearages,and to put forward？ corresponding measures. Methods Literature search and stratified random sampling were used to survey and analyze the general situation of medical treatment arrearages in Shandong Province and 106 medical institutions. Results The arrearage because of medical insurance, vulnerable groups, free med- ical service and lack of special compensation fund are the main reasons for the medical treatment arrearage. Conclusions The gov- ernment should perfect the medical security system, and enlarge the medical insurance coverage. The settlement way of medical insurance should be optimized, and the settlement efficiency of medical insurance should be improved. The medical relief system should be established to protect the health needs of vulnerable groups. Learning advanced experience to reform the free health care system. Medical debt compensation fund can be established to ensure the normal development of the medical institution.
