阴道镜下宫颈活检和宫颈环形电切术在诊断宫颈病变中的价值

摘　　要：目的探讨阴道镜下宫颈活检和宫颈环形电切术(LEEP)与两者联合诊断宫颈病变的价值。方法 600例经宫颈液基细胞学检查或人乳头瘤病毒检查异常者作为研究对象,进行阴道镜下宫颈活检与(或)LEEP,并比较。结果阴道镜下活检与LEEP术后诊断结果之间的总符合率为49.23%。对两种方法诊断为HSIL进行一致性检测,Kappa值为0.63。阴道镜下宫颈活检病理、LEEP术后病理为CINⅡ/Ⅲ级,与两者联合后病理一致性比较,Kappa值分别为0.81和0.83。结论阴道镜下宫颈活检与LEEP术病理检查均是有效诊断宫颈病变的方法,但两者联合可提高诊断准确性。Objective To investigate the value of the diagnosis of colposcopic cervical biopsy and cervical loop electrosurgical excision procedure （LEEP） for cervical lesions. Methods 600 cases whose liquid-based cervical cytology or human papillomavirus check were abnormal were studyed,the results of colposcopic cervical biopsy and （or） LEEP were compared. Results Total coincidence rate was 49.23% between the colposcopy biopsy and LEEP diagnostic resuits.The diagnostic pathology results of the two methods for the conformance testing of HSIL, Kappa=0.63. Colposcopic cervical biopsy and LEEP pathological resuhs compared with the combination of the two methods as CIN I1/11I level,Kappa values was 0.81 and 0.83, respectively. Conclusion Cervical biopsy under colposcopy and LEEP surgery for pathological examination are effective methods for diagnosis of cervical lesions,but the combination of two methods can improve the accuracy of diagnosis.
