萆苓痛风方对大鼠急性痛风性关节炎的实验研究

摘　　要：目的:评价萆苓痛风方对急性痛风性关节炎的抗炎镇痛作用及对血白细胞、血尿酸、血肌酐等的影响。方法:将70只雄性健康Wister大鼠随机分为模型组(A)、萆苓痛风方低剂量组(B)、萆苓痛风方中剂量组(C)、萆苓痛风方高剂量组(D)、别嘌呤醇组(E)、吲哚美辛组(F)、空白对照组(G)共7组,每组10只。观察不同时间点大鼠右侧踝关节周径、步态,治疗结束后测定血液中白细胞、尿酸、肌酐等的含量。结果:A组以造模后24h踝关节肿胀最为严重;B、C、D、E组以造模后6h踝关节肿胀最为严重,说明药物能减轻该模型的关节肿胀度;C组优于B组,C组与D组、E组疗效无明显差异(P<0.05),F组疗效优于C组、D组与E组;B、C、D、F组均能减轻大鼠的急性关节炎症,改善其步态;D组能减轻大鼠的急性炎症反应,效果优于其它组;C组降血尿酸作用优于B、D、F组,与E组相当;各组对血肌酐均无明显影响;A组可见滑膜下层大量中性粒细胞及淋巴细胞浸润;D组可见滑膜细胞不同程度增生,滑膜下层粘液变性,间质中性粒细胞减少甚至消失,可见散在淋巴细胞及浆细胞浸润。结论:中药萆苓痛风方对大鼠痛风性关节炎具有抗炎镇痛、降低血清中尿酸含量的作用,可用于急性痛风性关节炎的治疗,以C组效果最好。Objective： To evaluate the anti-inflammatory and analgesic effects of Biling Gout Decoction on rats with acute gouty arthritis and its effects oll white blood cell, blood uric acid and creatinine. Methods：70 male healthy Wister rats were randomly and equally divided into 7 groups： the model group （A）, the Biling gout Decoction group with low dose （B）, the Biling Gout Decoction group with medium dose （C）, the Biling Gout Decoction group with high dose （D）, the allopurinol group （E）, the indomethacin group （F） and the control group （G）, observing the right ankle joint circumference, gait of rats in different time and measuring the content of white blood cells, uric acid and creatinine in the blood after treatment.Results： Ankle swelling of group A was the most serious 24 h alter modeling, while those of groups B, C, D, E were the most serious 6 h after modeling, showing that the drug could reduce joint swelling of the model; group C was better than group B, having no significant difference from group D and group E （P 〈 0.05 ）; the curative effect of group F was better than that of group C, group D and group E; acute joint inflammation in rats could be reduced in groups B, C, D, E their gaits being improved; the acute inflammatory reaction in rats in group D could be reduced, the effect being superior to other groups; the reduce of blood serum uric acid in group C was superior to groups B, D, and F, being equal to that in group E; each group had no significant effect on blood creatinine; a large number of infiltrated neutrophils and lymphocytes in the lower synovial layer was visible in group A; in group D, hyperplastic synovial cells with different degree were visible and the mucoid degeneration occurred in the synovial layer, with stromal neutrophil decreasing or even disappearing, which could be seen scattering in the lymphocyte and plasma cell infiltration.Conclusion： Biling Gout Decoction, the traditional Chinese medicine had anti-inflammatory and analgesic effects on
