中药灌肠加口服治疗糖尿病肾病27例临床研究

摘　　要：目的观察中药灌肠加口服治疗糖尿病肾病的临床疗效。方法将46例糖尿病肾病患者随机分为2组,对照组19例予常规治疗,治疗组27例在对照组治疗基础上加中药灌肠加口服治疗。2组均14 d为1个疗程,共治疗2个疗程。检测2组治疗前后空腹血糖(FPG)、空腹胰岛素(FINS)、空腹C肽、糖化血红蛋白(HbAlc)、总胆固醇(TC)、甘油三酯(TG)、血肌酐及血尿酸水平变化情况,统计临床疗效。结果治疗组总有效率70.4%,对照组总有效率52.6%,2组总有效率比较差异有统计学意义(P<0.05),治疗组临床疗效优于对照组。2组治疗后各血糖、血脂指标与本组治疗前比较差异均无统计学意义(P>0.05),治疗组治疗前、后HbAlc均明显低于对照组同期(P<0.05)。治疗组治疗前血肌酐及血尿酸均显著高于对照组(P<0.05,P<0.01),治疗后血肌酐较本组治疗前明显下降(P<0.01),且治疗前后差值明显高于对照组(P<0.05),治疗组血肌酐下降幅度较对照组明显。结论中药灌肠加口服治疗糖尿病肾病可有效保护受损肾脏组织,延缓疾病发展,疗效确切,费用低廉。Objective To explore the effect of enema and oral medication of traditional Chinese medicine in treatment of diabetic nephropathy.Methods 46 patients with diabetic nephropathy were randomly divided into two groups.Patients in control group（n=19） were treated by routine therapy.Patients in treatment group（n=27） were treated by enema and oral medication of traditional Chinese medicine combined with routine therapy.The course was 14 days in two groups.The changes of FPG,FINS,C-peptide,HbAlc,TC,TG,serum creatine and blood uric acid before and after treatment were measured in two groups after two courses.Results The total effective rate in treatment group（70.4%） was higher than that in control group（52.6%,P0.05）.There was no significant difference between two groups on the levels of blood glucose and blood lipid after treatment（P0.05）.HbAlc in treatment group was lower than that in control group after treatment（P0.05）.Serum creatine after treatment was obviously decreased as compared with that before treatment in treatment group（P0.01）.The changes of serum creatine in treatment group was significantly decreased as compared with that in the control group after treatment（P0.05）.Conclusion Enema and oral medication of traditional Chinese medicine can effectively protect the damaged kidney tissue and delay disease development in treatment of nephropathy,and has definite efficacy and inexpensive cost.
