摘　　要：门诊病案姓名索引是医院信息系统的基本内容,也是识别患者的唯一资料,由于患者基本信息收集不全、姓名虚假,导致患者日后就医或住院出现麻烦。解决问题的对策是要从建立门诊姓名索引做起,严格执行患者就诊实名制,杜绝随意更改姓名索引、健全满足需求的索引卡、规范更改流程、强化质量管理、增强法律意识,以期更好地为广大患者,为医疗、教学、科研提供有价值的信息资料。Outpatient medical record name index is the basic content of hospital information system, is the only material to identify the patients. We can＇t follow up patients after discharge when information collection is not complete or the patient＇s name is false. We should put an end to change name index randomly, establish name index which satisfy patients needs, regulate change process, strengthen the quality management, enhance legal , in order to better service for the patients, and provide valuable information for medical treatment, teaching and scientific research
