摘  要:目的了解临床分离的耐亚胺培南鲍曼不动杆菌OXA-23基因的存在状况。方法收集我院临床分离的非重复耐亚胺培南鲍曼不动杆菌16株,采用纸片扩散法确定其耐药率,以PCR对亚胺培南耐药菌株进行OXA-23基因的检测并测序,序列与基因库比对。结果 16株耐亚胺培南鲍曼不动杆菌中,OXA-23基因阳性11株,阳性率为68.8%。随机抽取2株OXA-23基因阳性株进行测序后,经网上GenBank比对,与OXA-23标准株100%同源。结论分离菌株主要来源于痰标本,耐亚胺培南鲍曼不动杆菌OXA-23基因的携带率较高,分离菌株的耐药性可能与OXA-23型碳青霉烯酶基因有关。Objective To investigate the exsistence status of oxa-23 carbapenemase gene in Acinetobacter baumannii isolates resistant to imipenem,and to explore the drug-resistant mechanism. Methods 16 strains of nonduplicate clinical strains were isolated from Apr. 2012 to Sept. 2012, and K-B disk diffuse method was used insusceptibility testing. Regular polymerase chain reaction(PCR) was per- formed on the clinical isolates using primers specific for D-type carbapenemase OXA-23 gene, then the PCR product was sequened and we compared the sequences with the gene bank. Results There are 11 strains of Acinetobacter baumannii (68.8%) carrying with OXA- 23gene. Two strains with OXA-23 positive gene are randomly chosen for test and they are in 100% homogenous to OXA-23 standard strain. Conlusions The isolation strains mainly comes from sputum specimens, and the strains of imipenem-resistance Acinetobacter bau- mannii carry a relatively higher rate of OXA-23 gene. We consider the resistance of the isolates may be associated with OXA-23 carbapene- mases gene.
