改良中心静脉导管腹腔灌洗引流及护理干预治疗急性胰腺炎的临床研究

摘　　要：目的探讨采用改良中心静脉导管腹腔灌洗引流及护理干预治疗急性胰腺炎的临床研究。方法将笔者所在医院住院患者40例随机分为对照组及治疗组。对照组予以常规非手术保守治疗;治疗组在常规非手术治疗基础上,予以改良中心静脉导管腹腔灌洗引流治疗。观察并记录两组患者肛门首次排气时间、肠鸣音恢复时间、腹胀缓解时间、住院天数及治疗前后血淀粉酶的变化,以评价治疗效果。结果统计观察发现,治疗组患者肛门首次排气时间、肠鸣音恢复时间、腹胀缓解时间、住院时间均明显短于对照组,差异有统计学意义(P<0.01);治疗后第1、3、7天治疗组患者血淀粉酶下降程度均优于对照组,差异有统计学意义(P<0.01)。结论改良中心静脉导管腹腔灌洗引流对急性胰腺炎患者是一种有效、简单的疗法,值得临床进一步推广应用。Objective To explore improved central venous catheter drainage for abdominal irrigation used in the treatment and nursing care of patients with acute pancreatitis. Methods Forty inpatients in our hospital were divided into control group and treatment group randomly. The conventional non-operative therapy was used in the control group and continuous intraperitoneat drainage with modified central venous catheter was used on the basis of routine treatment in the treatment group. The recovery time of gurgling sound, first time of pass-gas by anus, release time of abdominal distension, hospitalization days and changes of hemodiastase pre and post treatment in two groups were observed and recorded to evaluate the effect of treatment. Results The Statistics and observation found that the recovery time of gurgling sound, first time of pass-,gas by anus, release time of abdominal distension and hospitalization days in the treatment group were all obviously shorter than those in the control group（P 〈 0~01）. The decreasing degree of hemodiastase in the treatment group on the 1st, 3rd and 7th days after treatment was all better than those in the control group（P 〈 0.01）. Conclusion Continuous intraperitoneal drainage with modified central venous catheter is an effective and simple therapy to patients with acute pancreatitis and worth further clinical application.
