摘　　要：目的探讨手部瘢痕挛缩畸形的手术治疗方法及效果。方法对笔者所在医院2008年1月～2012年8月收治的34例手部瘢痕挛缩畸形患者行Z字成形,瘢痕松解,皮片移植,皮瓣修复,支具牵引等治疗方法进行治疗,并观察治疗效果。结果 8例经Z字成形术治疗的患者中,5例(62.5%)手部功能完全正常,3例(37.5%)手部大部分功能恢复正常;12例游离植皮患者中,7例(58.33%)手部功能完全正常,3例(25.0%)手部大部分功能恢复正常,2例(16.67%)手部功能基本恢复;11例皮瓣移植患者中,4例(36.36%)手部功能完全正常,5例(45.45%)手部大部分功能恢复正常,2例(18.18%)手部功能基本恢复;3例牵拉治疗患者手部功能均恢复正常。结论对不同瘢痕挛缩特点的手部瘢痕挛缩畸形患者应选择不同的手术治疗方案,术后采用有效的康复训练与物理治疗可较好地恢复手部功能与形态。Objective To explore the surgery treatment and efficacy of hand scar contracture deformity. Methods 34 cases with hand scar contracture deformity in our hospital from January 2008 to August 2012 were selected and treated by Z-word angioplasty,scar release, skin graft, skin flap and brace traction.observed the treatment effect. Results The 8 patients treated by Z-word angioplasty,there were 5 cases＇ hand function were completely returned normally（62.5%）,3 cases＇hand function were mostly returned normally（37.5%）.The 12 patients treated by scar release,there were 7 cases＇hand function were completely normal（58.33%）.3 cases＇hand function were mostly returned normally（25.0%）,2 cases＇hand function were basic recovery（16.67%）.ll cases treated by skin flap,4 cases＇hand function were completely returned normally（36.36%）,5 cases＇hand function were mostly returned normally（（45.45%）,2 cases＇hand function were basic recovery（18.18%）.3 cases treated by brace traction,their hand function were all completely returned normally. Conclusion The hands with different characteristics of scar contracture deformity should choose different surgical treatment options,the postoperative effective rehabilitation and physical therapy can restore hand function and morphology.
