茵芩清肝汤治疗2型糖尿病伴非酒精性脂肪肝病的临床观察

摘　　要：目的探讨中药茵芩清肝汤对2型糖尿病(T2DM)伴非酒精性脂肪肝病(non-alcoholic fat-ty liver disease,NAFLD)的治疗效果。方法选取80例T2DM伴NAFLD患者,随机分为2组,在维持原糖尿病饮食、运动及降糖药物治疗方案不变的基础上,分别给予中药茵芩清肝汤(中药组)及血脂康(对照组)治疗,疗程3个月。观察两组患者治疗前、后血糖、胰岛素、胰岛素抵抗指数(HOMA-IR)及血脂、肝功能、肝纤维化指标及肝脏B超等方面的变化并进行比较。结果治疗后两组患者血糖、胰岛素、HOMA-IR、血脂、肝功能、肝纤维化指标及肝脏B超均较治疗前明显改善(P<0.05,P<0.01)。中药组总有效率为92.5%,显著优于对照组55.0%(P<0.01)。结论中药茵芩清肝汤能明显改善T2DM伴NAFLD患者的胰岛素抵抗、血脂、肝纤维病变、脂肪肝B超分度,具有较好的临床疗效。Objective To observe the therapeutic effect of Yinqin Qinggan decoction on type 2 diabetes melli- tus（T2DM）accompanying with non-alcoholic fatty liver disease（NAFLD）. Methods Eighty cases of T2DM pa- tients accompanying with NAFLD were randomly divided into treatment group and control group. While the origi- nal diabetes diet, exercise and hypoglycemic drug treatment programs were maintained in both groups, Yinqin Qinggan decoction was given to the treatment group and Xuezhikang was administered to the control group for three months. Changes in plasma glucose, insulin, HOMA-IR,lipids,liver function, serum fibrotic markers, and liv- er ultrasound before and after the treatment were observed, and clinical efficiency was evaluated. Results After the treatment for three months, plasma glucose, insulin, HOMA-IR, lipids, liver function, serum fibrotic markers, and liver ultrasound in the treatment group showed significant improvement（P^0.01 or P^0.05）. The total ef- fective rate was 92.5~ in Yinqin Qinggan decoction group and 55~ in the control group,with statistically signifi- cant difference between the two groups（P%0.01）. Conclusion Chinese medicine Yinqin Qinggan decoction could obviously improve plasma glucose,insulin, HOMA-IR, lipids, liver function, serum fibrotic markers and liver ultra- sound of T2DM patients accompanying with NAFLD,and had significant clinical efficacy.
