摘 要:目的探讨降钙素原(PCT)和C反应蛋白(CRP)在预测新生儿高胆红素血症合并呼吸道感染中诊断价值,从而实现早期治疗的目的。方法选取医院2008年1月-2011年12月就诊的150例新生儿高胆红素血症患儿(入院时并不存在感染),入院时即检测血清中的PCT和CRP,根据入院7d后患儿是否发生呼吸道感染分为呼吸道感染组和非感染组,进行回顾性分析。结果呼吸道感染组血清中PCT表达率为86.0%,非感染组PCT表达率为5.0%(P<0.01);CRP表达率呼吸道感染组为64.0%,非感染组为8.0%(P<0.01);PCT阳性组出现呼吸道感染症状和体征时间为(4.318±0.515)d,高于CRP阳性组的(2.324±0.262)d,差异有统计学意义(P<0.05)。结论 PCT和CRP可早期指导诊断新生儿高胆红素血症患儿发生呼吸道感染,有助于患儿的早期治疗和康复。OBJECTIVE To study the application value of PCT and CRP in the prediction of neonatal hyperbillirubinemia patients complicated with respiratory tract infection so as to treat the disease in the early stage. METHODS A total of 150 cases of neonatal hyperbilirubinemia patients in the hospital from Jan 2008 to Dec 2011 were selected, their PCT and CRP in serum were tested at the time of admission. The patients were divided into the respiratory tract infection group and the non-infection group according to the infection status 7 days after the enrollment ,then retrospective analysis was performed. RESULTS The positive rate of expression of PCT in the respiratory tract infection group was 86.0% ,and 5.0% in the non-infection group (P〈0.01), the positive rate of CRP in the respiratory tract infection group was 64. 0%, 8. 0% in the non-infection group (P〈0.01) ; the respiratory tract infection symptoms of the positive PCT group and signs of time was (4. 318 ± 0. 515) d, higher than (2. 324 ± 0. 262) d of the CRP-positive group, the difference was statistically significant (P〈0. 05). CONCLUSION PCT and CRP can be the guidance for the early diagnosis of respiratory infection that occurs in the neonatal hyperbilirubinemia patients, and contribute to the children's early treatment and rehabilitation..
