会阴侧切术切口感染危险因素分析及预防控制措施

摘　　要：目的探讨引起会阴侧切术切口感染的危险因素,制定预防与控制感染的有效措施。方法根据2010年8月-2011年8月经阴道分娩行会阴侧切术的5914例足月妊娠产妇的临床资料,对可能引起切口感染的因素进行单因素分析,从各个环节加以预防控制。结果会阴切口感染60例,感染率1.01%,细菌培养结果主要以粪肠球菌为主,检出28株占46.67%,其次是大肠埃希菌13株和金黄色葡萄球菌8株,分别占21.67%和13.33%;引起会阴切口感染的主要因素与合并基础疾病、胎膜早破、阴检肛查过频、缝合技术、产程过长、卫生宣教欠缺等有关。结论会阴侧切术切口感染与多因素有关,应针对相关危险因素预防和控制切口感染,积极治疗各种合并疾病,阴检肛查根据实际需要慎重选择,及时处理胎膜早破、提高会阴侧切缝合技巧、缩短产程,可以有效降低侧切感染率。OBJECTIVE To explore the risk factors for incisions infections caused by the episiotomy so as to find effective measures to prevent and control the infections. METHODS According to the clinical data of 5914 pregnant women undergoing episiotomy for delivery via vagina from Aug 2010 to Aug 2011, the univariate analysis of the factors that may cause the incision infections was performed, the prevention and control of the infections were started from the various links. RESULTS There were 60 cases of perineal incision infections with the incidence of 1.01%, the bacterial culture results showed that totally 28 strains of Enterococcus faecalis were detected, followed by 13 strains of Escherichia coli and 8 strains of Staphylococcus aureus; the underlying diseases, premature rupture of fetal membrane, frequent vaginal and rectal check, suture techniques, long duration of delivery and the insufficient health care promotion were the main＇risk factors for perineal incision infections. CONCLUSION Episiotomy incision infections are related to multiple factors, it should focus on prevention and control of risk factors associated with incision infections, pay high attention to the vaginal and rectal check, treat the premature rupture of fetal membrane in time, improve the episiotomy techniques and shorten the birth process so as to effectively reduce the incidence of perineal incision infections.
