手助腹腔镜肾切除术15例临床体会

摘　　要：目的探讨手助腹腔镜肾切除术的方法和临床应用价值。方法运用手助腹腔镜技术对12例肾癌患者行根治性肾切除术;3例巨大肾积水患者行单纯肾切除术。观察手术时间、术中出血量、胃肠功能恢复时间和住院时间。结果 15例手术全部成功。其中手术时间100～210min,平均165min,术中出血量40～200mL,平均112mL。术后住院3～7d,平均5d。结论与开放手术及传统腹腔镜手术相比,手助腹腔镜手术具有创伤小、出血少、恢复快等优点,为复杂肾脏切除手术提供了新的相对便利的微创术式。Objective To evaluate the effect and clinical application of hand-assisted laparoscopic nephrectomy. Methods 15 patients underwent hand-assisted laparoscopy： radical nephrectomy was performed on 12 cases, and simple nephrectomy on 3 cases. The operating time, blood loss, postoperative intestinal function recovery and hospital stay were observed. Results All cases were performed successfully. The mean operation time was 165 minutes （100 -210min）,and the mean blood loss was 112 mL （40 -200mL）. The mean postoperative hospital stay was 5 days （3-7 days）in all cases. Conclusion With the advantages of minimal invasion, less blood loss, faster recovery, hand-assisted laparoscopy is worth popularizing in urologic clinical practice, especially in difficult nephrectomy.
