消痰解郁方治疗消化系统癌症并发抑郁症随机对照临床研究

摘　　要：目的评价消痰解郁方治疗消化系统癌症并发抑郁症的有效性与安全性。方法采用前瞻性、完全随机、平行对照方法,将120例消化系统癌症并发抑郁症患者分为治疗组(消痰解郁方,60例)、中药对照组(逍遥丸,30例)和西药对照组(盐酸氟西汀,30例),治疗6周,治疗前后测评患者KPS评分、抑郁自评量表(SDS)、汉密顿抑郁量表(HAMD),并检测血清T细胞亚群及NK细胞水平。结果消痰解郁方可以改善患者的临床症状、提高KPS评分,降低SDS和HAMD分值;升高血清NK细胞、CD3和CD4/CD8,降低CD8,差异有统计学意义(P<0.05,P<0.01),总有效率为82.46%(47/60),优于中药和西药对照组(P<0.05,P<0.01),且无不良反应。结论消痰解郁方治疗消化系统癌症并发抑郁症安全有效。Objective To observe the efficacy and safety of Xiaotanjieyu rescription（XTJYF） on patients with digestive cancer concurrent depression.Methods With prospective,completely randomized,parallel control method,120 cases of digestive cancer concurrent depression were divided into 3 groups：treatment group（treatment with XTJYF,n =60）,TCM controlled group（treatment with Xiaoyaowan Pill,n =30） and western medicine controlled group（treatment with Fluoxetine Collocystis,n = 30）.Before and after 6 weeks’ treatment,Karnofsky performance status（KPS）,Self-Rating Depression Scale（SDS） and Hamilton Rating Depression Scale（HAMD,24） of patients were assessed,the level of serum T cells and NK cells were also detected.Results After treatment of XTJYF,clinical symptoms were improved,the scores of KPS,SDS and HAMD and level of serum CD8were decreased,level of NK cell,CD3and CD4/CD8were increased,with significant difference（P 0.05 or P 0.01）.The total effective rate was 82.46%（47/60）,better than that in the other two groups（P 0.05 or P 0.01）,without side effects.Conclusions XTJYF is safe and effective on patients with digestive cancer concurrent depression.
