摘 要:目的探讨瑞芬太尼对。肾脏缺血再灌注(I/R)损伤大鼠核苷酸结合寡聚化结构域1(NOD1)mRNA表达的影响。方法健康成年雄性SD大鼠60只,体重220~250g,采用随机数字表法,将其随机分为3组(n=20):假手术组(S组)、缺血再灌注组(I/R组)和瑞芬太尼组(R组)。I/R组和R组采用夹闭双侧。肾动脉45min再灌注法制备肾脏I/R损伤模型。R组于缺血前15min时静脉输注瑞芬太尼1.0μg·kg^-1·min^-1至再灌注30min,S组和I/R组用等容量生理盐水替代。于缺血前15min及再灌注3、6、24h时取肾组织,观察。肾组织病理学结果,并计数中性粒细胞(PMN),采用流式细胞术舣染法检测细胞凋亡率,采用RT—PCR法测定NOD1mRNA表达水平。结果与S组比,I/R组再灌注各时点肾组织NOD1mRNA表达水平上调,细胞凋亡率和PMN计数升高,R组再灌注各时点肾组织细胞凋亡率和PMN计数升高,再灌注6、24h时NODlmRNA表达上调(P〈0.01);与I/R组比较,R组再灌注各时点肾组织NOD1mRNA表达水平下调,细胞凋亡率和PMN计数降低(P〈0.05或0.01),且肾组织病理学损伤减轻。结论瑞芬太尼通过下调NOD1mRNA表达,抑制炎性反应和细胞凋亡,从而减轻大鼠肾脏I/R损伤。Objective To investigate the effect of remifentanil on nucleotide-binding oligomerization do- main I (NOD1) mRNA expression in rats with renal ischemia-reperfusion (I/R) injury. Methods Sixty male Sprague-Dawley rats, weighing 220-250 g, were randomly divided into 3 groups ( n = 20 each) : sham operation group (S group), I/R group and remifentanil group (R group). Renal ischemia was induced by occlusion of bilat- eral renal arteries for 45 min followed by 24 h reperfusion in groups I/R and R. Remifentanil 1.0 μg·kg^-1·min^-1 was infused until 30 min of reperfusion starting from 15 min before ischemia in group R, while the equal volume of normal saline was given instead in S and I/R groups. The animals were sacrificed at 15 min before ischemia and at 3, 6, 24 h of reperfusion and the kidneys were removed for microscopic examination and polymorphonuclear leuko- cyte (PMN) count and for measurement of NOD1 mRNA expression (by RT-PCR). The apoptotic rate was deter- mined by flow cytometry double staining method. Results Compared with group S, NOD1 mRNA expression was up-regulated, and the apoptotic rate and PMN count were significantly increased at each time point during reperfu- sion in group I/R, and the apoptotic rate and PMN count were significantly increased at each time point during reperfusion, and NOD1 mRNA expression was up-regulated at 6 and 24 h of reperfusion in group R ( P 〈 0.01 ). Compared with I/R group, NOD1 mRNA expression was down-regulated, and the apoptotic rate and PMN count were significantly decreased at each time point during reperfusion ( P 〈 0.05 or 0.01 ), and the pathological changes were significantly attenuated in group R. Conclusion Remifentanil can reduce the renal I/R injury bydown-regulating the expression of NOD1 mRNA and inhibiting inflammatory response and apoptosis.
