精索护垫补片在腹股沟疝修补术中的应用研究

摘　　要：目的对比应用精索护垫补片与标准型聚丙烯补片在青壮年男性腹股沟疝患者中行无张力疝修补术,探讨精索护垫补片在青壮年男性腹股沟疝治疗中的应用价值。方法采用前瞻性随机分组,将符合条件的患者分为两组,试验组使用精索护垫补片,对照组使用标准型聚丙烯补片,每组各30例患者。记录两组患者手术前后的临床参数及术后各种并发症的发生例数,并对数据进行统计学分析。结果经统计分析,两组患者在手术时间、术后住院日数等方面差异无显著性。术后随访12~18个月,两组均未出现伤口感染、复发和睾丸的肿大/萎缩。试验组术后有3例出现阴囊积液,没有局部异物感和射精痛发生;对照组术后有3例发生阴囊积液,6例出现局部异物感和射精痛。经统计学分析,两组在术后局部异物感及射精痛的发生率方面差异有显著性（P〈0.05）。结论精索护垫补片在青壮年男性腹股沟疝无张力修补术中的应用安全有效,可减少术后局部异物感及射精痛的发生。【Objective】 To compare the effectiveness of protective pad of spermatic cord patch and polypropylene mesh in lichtenstein inguinal herniorrhaphy for male patients aged 18 to 40 years.【Methods】 Sixty patients with primary unilateral inguinal hernia were randomly assigned to experimental or control group.In experimental group,protective pad of spermatic cord patch was used in lichtenstein herniorrhaphy while polypropylene mesh was used in the control group.Operation time,postoperative length of stay,postoperative complications and recurrence rate were recorded and analyzed.【Results】 There was no between-group difference in operation time or postoperative length of stay（P 0.05）.During the follow-up time ranged from 12 months to 18 months,there was no hernia recurrence,wound infection or testicular swelling/atrophy in the study.In the two groups,there were 3 patients suffered scrotal hydrocele.In the experimental group,no patients experienced local foreign body sensation and painful ejaculation as compared with 6 patients in the control group（P 0.05）.【Conclusions】 The use of protective pad of spermatic cord patch for tension-free inguinal herniorrhaphy in the treatment of male patients aged 18 to 40 years is effective and safe.
