前列腺增生尿动力学分析(附600例报告)

摘　　要：目的研究前列腺增生症(BPH)患者术前尿动力学(UDS)检查,探讨尿动力学检查对BPH诊断、术前评估的价值。方法对600例前列腺增生症患者行UDS检查并进行尿动力学评估,为前列腺增生症患者手术治疗提供理论依据。结果 600例均存在膀胱出口梗阻(BOO),单纯(BOO)不伴明显膀胱功能障碍者303例(50.5%),膀胱过度活动102例(15.45%),低顺应性膀胱109例(18.16%),低顺应性伴膀胱过度活动49例(8.16%),逼尿肌收缩力减弱83例(13.83%),逼尿肌外括约肌协同失调3例(0.5%)。结论前列腺增生症患者尿动力学检测可同时评估膀胱和尿道功能情况,对BPH患者BOO确诊、判断逼尿肌功能、选择治疗方法及预测疗效等均有重要的参考价值。【Objective】 To study the urodynamic analysis of benign prostatic hyperplasia,to evaluate the value of urodynamics in the diagnosis and surgery of the patients with benign prostatic hyperplasia.【Methods】 Urodynamic tests and urodynamic evaluation of the treatment were made to 600 cases with benign prostatic phyperplasia so as to provide the theoretical basis for surgical treatment.【Results】 All the 600 cases suffered from bladder outlet obstruction（BOO）,303 cases had pure BOO without bladder dysfunction（50.5%）,102 cases suffered from bladder instability（15.45%）,109 cases had low compliance bladder（18.16%）,49 cases suffered from unstable bladder with low compliance（8.16%）,83 cases had weak detrusor contraction（13.83%）,and 3 cases had detrusor sphincter dysfunction（0.5%）.【Conclusions】 Urodynamic test can evaluate the function of bladder and urethra and have very important value to dignoes BOO of BPH,appraise the function of detrusor muscle,select and predict the therapy.
