摘 要:目的探讨清下解胰方治疗重症胰腺炎的作用机理。方法将85只雄性大鼠随机分为假手术、模型组、清下解胰方组、清下组、化瘀组、疏肝组,通过胆胰管逆行注射法建立重症胰腺炎大鼠模型,所有组别于术前12 h、术后及术后11 h干预,并于术后12 h后取材检测Toll样受体4(TLR4)mRNA、核因子-κB(NF-κB)mRNA、IL-1β、病理评分并行各指标之间相关性分析。结果清下解胰方组大鼠病理损伤明显少于模型组,拆方各组较模型组也有不同程度的减少。血清IL-1β:清下解胰方组和清下组较模型组均有显著降低。TLR4 mRNA:清下解胰方组较模型组显著降低;NF-κBmRNA:清下解胰方组和清下组较模型组均显著降低。病理评分与TLR4和NF-κB呈正相关,但与IL-1β不相关;TLR4与NF-κB和IL-1β均呈正相关。结论清下解胰方可能通过抑制TLR4/NF-κB信号通路而起到治疗作用。清下解胰方主要通过其清热解毒、通腑泻下和活血化瘀而起到治疗重症胰腺炎的作用。Objective To investigate the mechanism of Qingxia Jieyi Fang in the treatment of severe acute pancreatitis (SAP). Methods Male rats (n = 85 ) were randomly divided into sham-operation group (SO group), model group (SAP group), Qingxia Jieyi Fang group (QXJY group), Qingxia Fang group (QX group), Huayu Fang group (HY group) and Shugan Fang group (SG group). The rat model of SAP was established by retrograde injection of sodium taurocholate into the biliopancreatic duct. All groups were interfered 12 hours before, after and 11 hours after the operation. The expressions of TLR4 mRNA and NF-KBp65 mRNA, level of interleukin-1 ~ (IL-1 [3) and pathological scores were detected 12 hours after the operation, and the correlation among all indexes was analyzed. Results The pathological injury was alleviated significantly in QXJY group, and alleviated in groups with disassembled formulas compared with model group. The level of IL-κ decreased significantly in QXJY group and QX group compared with model group. The expression of TLR4 mRNA decreased significantly in QXJY group compared with model group, and expression of NF-κBp65 mRNA decreased significantly in QXY groupand Qx group compared with model group. The pathological scores were positively correlated to TLR4 and NF-KBp65 but not to IL-1β, and TLR4 was positively correlated to NF-KBp65 and IL-1β. Conclusion Qingxia Jieyi Fang may play a therapeutic role through inhibiting the signal pathway of TLR4/NF-KB, and it has the actions of clearing heat and removing toxin, relaxing bowels and purgation, and activating blood and resolving stasis.
