摘　　要：股骨头坏死(ONFH)一直是骨科常见且治疗困难的疾病。此病多见于中青年,晚期患者目前只能接受全髋关节置换术,加之伴随翻修的风险,会严重影响患者的生活。在疾病早期采取积极的保头治疗,可有效阻止和限制坏死的进展,是中青年患者首选治疗方式。近10余年来,ONFH保头治疗日趋多样,各具特色。关键在于如何修复和重建骨质,重建并改善股骨头内的血运,保留股骨头的完整性,保持股骨头的解剖承重力学关系,引起了许多学者的广泛关注与研究。Osteonecrosis of the femoral head（ONFH） has long been the common disease of orthope- dics which is difficult to cure. This disease is popular in the middle-aged and the youth, and patients of ad- vanced stage have to receive total hip replacements （THR）at present, furthermore, there is risk concerning overhauling, which severely affects the llfe of the patients. It is efficient to hamper and constrain the devel- opment of necrosis to conduct active treatment to reserve the femoral head in the early stage of the disease, which is the optimal treatment for the middle-aged and the young patients. Over the last decade, there are more femoral head retention treatments of ONFH with own hallmarks gradually. The key is how to repair and reconstruct bone, reconstruct and ameliorate inner blood flow of the femoral head, retain the integrity of the femoral head, and sustain mechanics of the anatomical weight bearing of the femoral head, which causes general attention of substantial scholars.
